Pharmacological properties
Pharmacodynamics
Bismuth tripotassium dicitrate is an antiulcer and hepatoprotective agent that has a bactericidal effect against the Helicobacter pylori bacteria. Also has astringent and anti-inflammatory effects.
After taking the drug inside in the acidic environment of the stomach, precipitation of insoluble bismuth oxychloride and citrate occurs, as well as the formation of chelated compounds with a protein substrate in the form of a protective film on the surface of erosions and ulcers. Thus, a protective layer is formed, which for a long time covers the affected areas of the mucous membrane of the gastrointestinal tract (GIT) from the effects of aggressive factors.
Bismuth tripotassium dicitrate stimulates the activity of cytoprotective mechanisms, increases the secretion of bicarbonate, the formation of mucus, the synthesis of prostaglandin E and the resistance of the gastrointestinal mucosa to the effects of hydrochloric acid, bile salts, pepsin and enzymes. Promotes the accumulation of epidermal growth factor in the defect zone. Reduces the activity of pepsinogen and pepsin.
Pharmacokinetics
After oral administration of bismuth tripotassium dicitrate from the gastrointestinal tract is practically not absorbed.
It is excreted mainly in the feces. A small amount of the drug that enters the plasma is excreted by the kidneys.
Indications for use
- functional dyspepsia due to inorganic gastrointestinal diseases;
- irritable bowel syndrome, manifested mainly by symptoms of diarrhea;
- peptic ulcer of the stomach and duodenum in the acute phase, including caused by Helicobacter pylori;
- chronic gastritis and gastroduodenitis in the acute phase, including those associated with the bacterium Helicobacter pylori.
Contraindications
- chronic renal failure;
- the period of pregnancy and breastfeeding;
- children under 4 years of age;
- simultaneous use of other preparations containing bismuth;
- known hypersensitivity to any component of bismuth tripotassium dicitrate.
Bismuth tripotassium dicitrate, instructions for use: method and dosage
Bismuth tripotassium dicitrate tablets are indicated for oral administration and should be swallowed whole (crushing or chewing is not allowed) with sufficient liquid, with the exception of milk and fruit juices.
The optimal dose and duration of the course of therapy are determined by the attending physician, depending on the indication and the nature of the course of the disease.
Recommended dosage regimens of bismuth tripotassium dicitrate:
- adults and adolescents from 12 years old: 1 tablet 4 times a day (30 minutes before breakfast, lunch and dinner, the last intake - at night) or 2 tablets 2 times a day (30 minutes before breakfast and dinner);
- children 8-12 years old: 1 tablet once a day (30 minutes before breakfast and dinner);
- children 4-8 years: the daily dose is 8 mg / kg, which corresponds to 1 or 2 tablets in 1 or 2 doses, respectively (30 minutes before meals).
Typically, the course of therapy lasts 4–8 weeks.
For the eradication of the bacteria Helicobacter pylori Bismuth tripotassium dicitrate is advisable to prescribe in combination with antibacterial agents that have an anti-Helicobacter effect.
Side effects
The following side effects, which were observed in patients treated with Bismuth tripotassium dicitrate, are distributed in frequency as follows: ≥ 1/10 - very often, from ≥ 1/100 to <1/10 - often, from ≥ 1/1000 to < 1/100 - infrequently, from ≥ 1/10 000 to <1/1 000 - rarely, <1/10 000 - very rare:
- disorders of the gastrointestinal tract: very often - staining of feces in black; infrequently - diarrhea / constipation, nausea, vomiting;
- diseases of the nervous system: very rarely with prolonged use of high doses of the drug - encephalopathy due to the accumulation of bismuth in the central nervous system;
- allergic reactions: infrequently - itching, skin rashes; very rarely - anaphylactic reactions.
Known adverse reactions are reversible, quickly disappear after the end of the intake of Bismuth tripotassium dicitrate. In case of aggravation of any of the listed phenomena or the development of other adverse reactions not indicated in the instructions, you should consult a doctor.
Overdose
After the end of the treatment course with the drug in the recommended doses, the plasma concentration of bismuth does not exceed 58 μg / l. Intoxication develops when the level of the active substance in the plasma is more than 100 μg / l, that is, with prolonged use of overestimated doses of the drug or short-term use of the drug in doses that exceed the recommended dozens of times.
Signs of bismuth poisoning: dyspeptic disorders, inflammation of the oral mucosa, rash, characteristic darkening of the gums in the form of blue lines. With prolonged use of excessive doses - impaired renal function. After discontinuation of the drug, these symptoms are completely reversible.
There is no specific antidote for bismuth tripotassium dicitrate. You should rinse the stomach and take enterosorbent. Saline laxatives are recommended. Further, if necessary, symptomatic and supportive (to improve kidney function) treatment is carried out. With the development of impaired renal function, accompanied by a high content of bismuth in the blood plasma, complexing agents may be prescribed - dimercaptosuccinic and dimercaptopropanesulfonic acids. With severe functional impairment of the kidneys, hemodialysis is performed.
special instructions
The maximum duration of treatment is 8 weeks and should not be exceeded. Also, you cannot independently increase the doses established for children and adults.
If there is no improvement in the condition as the drug is taken, a medical consultation is required.
Within 2 months after the end of the course, it is not recommended to take any products containing bismuth (for example, Vikair or Vikalin).
Against the background of bismuth therapy, a slight darkening of the tongue is possible, as well as staining of feces in a dark color.
Patients should be informed that alcohol consumption during treatment is undesirable.
Influence on the ability to drive vehicles and complex mechanisms
Special studies on the effect of bismuth tripotassium dicitrate on psychomotor and cognitive functions have not been conducted. Given the side effect profile, adverse effects are unlikely.
Application during pregnancy and lactation
Bismuth tripotassium dicitrate is contraindicated during pregnancy and lactation. Breastfeeding women should transfer the baby to artificial feeding if drug therapy is vital.
Pediatric use
The drug is not prescribed for children under 4 years of age. At an older age, it is used according to indications, taking into account age-related dosages.
With impaired renal function
In case of impaired renal function, Bismuth tripotassium dicitrate tablets are used with caution.
For violations of liver function
The drug is not metabolized in the liver, therefore, its pharmacokinetics in hepatic failure have not been studied.
Use in the elderly
There is no data indicating the need to adjust the dose of bismuth tripotassium dicitrate in elderly patients.
Drug interactions
Within 30 minutes before and 30 minutes after taking the drug, you should refrain from eating food, fruits, fruit juices and milk, as well as from taking antacid drugs, since they can reduce the effectiveness of bismuth.
