Release form and composition
The drug is produced in the form of eye drops 0.25% and 0.5% (in dropper bottles of 5 ml).
1 ml of eye drops contains:
- Active ingredient: timolol - 2.5 mg and 5 mg;
- Excipients: benzalkonium chloride, povidone, disodium edetate dihydrate, sodium dihydrogen phosphate dihydrate, sodium monohydrogen phosphate dodecahydrate, distilled water.
Pharmacological properties
Pharmacodynamics
The active substance of Arutimol - timolol - belongs to non-selective blockers of beta-1 and beta-2 adrenergic receptors. Internal sympathomimetic and membrane stabilizing activity is uncharacteristic for him.
When applied topically in the form of eye drops, Arutimol lowers both normal and increased intraocular pressure due to inhibition of tear fluid production. Timolol does not alter accommodation and pupil size. The effect of the drug becomes noticeable 20 minutes after instillation into the conjunctival cavity. Intraocular pressure decreases as much as possible after 1-2 hours, and prolonged action is observed within 24 hours after administration.
Pharmacokinetics
When applied topically, timolol penetrates the cornea at a high speed. After instillation, the maximum concentration of the substance in the aqueous humor of the anterior chamber of the eye is recorded after 1-2 hours.
80% of timolol, used in the form of eye drops, enters the systemic circulation through absorption through the vessels of the conjunctiva, lacrimal tract and nasal mucosa. The metabolites of the compound are excreted mainly through the kidneys. In newborns and young children, the content of timolol as the active component of the drug significantly exceeds its maximum level in blood plasma in adult patients.
Indications for use
- Increased intraocular pressure (ocular hypertension);
- Angle-closure glaucoma (as an additional means to reduce IOP simultaneously with miotics);
- Secondary glaucoma;
- Open-angle glaucoma;
- Congenital glaucoma (if other therapeutic measures are not effective enough).
Contraindications
- Sinus bradycardia;
- Chronic obstructive airways disease (including bronchial asthma);
- Severe heart failure;
- AV block II or III degree;
- Corneal dystrophy;
- Cardiogenic shock;
- Allergic reactions with generalized skin rashes;
- Severe atrophic rhinitis;
- Hypersensitivity to the active substance (timolol) or the auxiliary components of the drops.
Carefully:
- Severe cerebrovascular insufficiency;
- Pulmonary insufficiency;
- Diabetes;
- Myasthenia gravis;
- Chronic heart failure;
- Thyrotoxicosis;
- Hypoglycemia;
- Simultaneous use with other beta-blockers.

Instructions for the use of Arutimol: method and dosage
The drug is instilled into the conjunctival sac of the affected eye 1 drop twice a day. After stabilization of the state, the frequency of application of drops is reduced to 1 time per day.
The product is intended for long-term use. It is possible to interrupt the prescribed treatment or change the dosage only as directed by a doctor.
Side effects
When using Arutimol eye drops, various side effects may occur:
- Respiratory system: bronchospasm, shortness of breath, pulmonary insufficiency;
- Organ of vision: photophobia, corneal hyposthesia, irritation, hyperemia of the conjunctiva and skin of the eyelids, punctate superficial keratopathy, ptosis, burning and itching in the eyes, lacrimation, diplopia, edema of the corneal epithelium, dry eyes;
- Central and peripheral nervous system: dizziness, headache, depression, weakness, paresthesia;
- Cardiovascular system: lowering blood pressure, bradycardia, heart failure, cardiac arrest, bradyarrhythmia, collapse, AV block, transient cerebrovascular accident;
- Digestive system: diarrhea, nausea.
In the presence of hypersensitivity, the development of allergic reactions in the form of eczema and urticaria is possible. Also, during therapy, rhinitis, alopecia and sexual dysfunctions may appear.
The main symptoms of an overdose are nausea, dizziness, bronchospasm, headache, bradycardia, arrhythmia, vomiting. For treatment, it is necessary to rinse the eyes with saline or water and, if necessary, carry out symptomatic therapy.
Overdose
Overdose may cause systemic effects typical of beta-blockers. Their symptoms are nausea, vomiting, headache, bronchospasm, lowering blood pressure, bradycardia, heart failure. In this case, immediately rinse the eyes with plenty of water or 0.9% sodium chloride solution. If necessary, symptomatic therapy is prescribed.
special instructions
To avoid deposits of preservative in contact lenses, they should be removed before the procedure and installed 15 minutes after instillation.
Do not simultaneously instill two beta-blocker drugs in the eyes.
In the case of an upcoming operation with the use of general anesthesia, the use of Arutimol should be canceled within two days.
Immediately after instillation, there may be a decrease in vision clarity and a slowdown in the speed of psychomotor reactions, which must be taken into account when performing potentially hazardous types of work. The effect is enhanced with alcohol consumption.
Pediatric use
According to the instructions, Arutimol is not recommended for children under 18 years of age.
Drug interactions
With the simultaneous use of eye drops, which include epinephrine, pupil dilation may be observed.
The decrease in intraocular pressure, considered a specific effect of the drug, is enhanced when combined with eye drops containing pilocarpine and epinephrine. Do not instill in the eyes at the same time two drugs that are beta-blockers.
The combination of Arutimol with beta-blockers, reserpine and calcium channel blockers can lead to increased side effects such as slowing heart rate and lowering blood pressure.
The simultaneous use of the drug with oral hypoglycemic drugs or insulin sometimes causes hypoglycemia.
Timolol enhances the effect of muscle relaxants, therefore, the drug is canceled 48 hours before the proposed surgery using general anesthesia.
