Altevir is a drug with antiproliferative, antitumor, immunomodulatory, antiviral effects.

Release form and composition

Altevir is released in the form of a solution for injection: transparent, colorless (in ampoules of 0.5 or 1 ml, 5 ampoules in contour plastic packaging, in a cardboard box of 1 or 2 packages; in bottles of 0.5 or 1 ml, in a cardboard pack 1 bottle or 5 bottles in contour plastic or cell packs, in a cardboard box 1 pack; in syringes of 0.5 or 1 ml, 1 or 3 syringes in contour plastic packaging, in a cardboard box 1 or 3 packages).

Composition of 0.5 or 1 ml of injection solution:

  • Active ingredient: human recombinant interferon alpha-2b - 1, 3, 5, 10 or 15 million IU (international units);
  • Additional components: sodium chloride, sodium acetate, disodium salt of ethylenediaminetetraacetic acid, dextran 40, tween-80, water for injection.

Indications for use

Altevir is prescribed for adults simultaneously with other drugs in the treatment of the following diseases:

  • Chronic viral hepatitis C, occurring without symptoms of liver failure (in the form of monotherapy or in combination with ribavirin);
  • Chronic viral hepatitis B without signs of liver cirrhosis;
  • Genital warts;
  • Laryngeal papillomatosis;
  • Multiple myeloma, hairy cell leukemia, progressive kidney cancer, chronic myeloid leukemia, non-Hodgkin's lymphoma, Kaposi's sarcoma with AIDS, melanoma.

Contraindications

  • Renal / hepatic impairment in severe course (including associated with metastases);
  • A history of severe cardiovascular disease (recent myocardial infarction, uncontrolled chronic heart failure, severe cardiac arrhythmias);
  • Hepatitis in a chronic course with decompensated cirrhosis of the liver and in patients receiving or recently receiving immunosuppressive therapy (except for a completed short course of treatment with glucocorticosteroids);
  • Severe myelodepression;
  • Epilepsy, as well as severe dysfunctions of the central nervous system, especially expressed by suicidal attempts and thoughts, depression (including a history);
  • Hypercoagulability, including thrombophlebitis, pulmonary embolism;
  • Autoimmune hepatitis or other autoimmune diseases;
  • Diseases of the thyroid gland that cannot be controlled by conventional therapeutic methods;
  • The use of drugs with immunosuppressive effects after transplantation;
  • Decompensated diabetes mellitus;
  • Decompensated lung disease, including chronic obstructive pulmonary disease;
  • Pregnancy and breastfeeding period;
  • Hypersensitivity to drug components.

Method of administration and dosage

Altevir is administered subcutaneously (subcutaneously), intramuscularly (intramuscularly), or intravenously (intravenously).

A doctor should start therapy. In the future, with his permission, the patient can self-administer a maintenance dose (with subcutaneous or intramuscular injection).

Recommended dosage regimen (route of administration / single dose / frequency of application / duration of the course):

  • Chronic hepatitis B: i / m or s / c / 5-10 million ME / 3 times a week / 16-24 weeks. If, after 3-4 months of using Altevir, there is no positive trend (according to the DNA of the hepatitis B virus), the therapy is canceled;
  • Chronic hepatitis C: i / m or s / c / 3 million ME / 3 times a week / 24-48 weeks. With a recurrent course of the disease, as well as in patients who have not previously received treatment with interferon alfa-2b, the effectiveness of Altevir increases when combined with ribavirin. A course of 48 weeks is prescribed for patients with chronic hepatitis C and the first genotype of the virus with a high viral load in cases where by the end of the first 24 weeks of therapy the hepatitis C virus RNA is not detected in the blood serum;
  • Laryngeal papillomatosis: n / k / 3 million ME / m 2 / week 3 times / 6 months. Therapy is prescribed after surgical / laser removal of tumor tissue. When choosing a dose, it is necessary to take into account the tolerability of the drug;
  • Hairy cell leukemia: sc / 2 million ME / m 2 (after or without splenectomy) / 3 times a week / from 1-2 to 6 months. The dosage regimen must be adhered to constantly, unless there is a rapid progression of the disease or the onset of symptoms of severe intolerance to Altevir;
  • Chronic myeloid leukemia: s / c / 4-5 million IU / m 2 / daily / individually. To maintain the number of leukocytes, the dose of Altevir can vary from 0.5 to 10 million IU / m 2. In cases where treatment allows to achieve control of the number of leukocytes to maintain remission prescribed maximum tolerated dose (4-10 million IU / m 2 daily). In cases where after 8-12 weeks treatment does not lead to a clinically significant decrease in the number of leukocytes or partial hematological remission, the drug is canceled;
  • Non-Hodgkin's lymphoma: n / a / 5 million IU / m 2 / week 3 times / 2-3 months. Altevir is prescribed as an adjuvant therapy in combination with standard chemotherapy regimens. Depending on the tolerability of the drug, dose adjustment is possible;
  • Melanoma: in / in / 15 million IU / m 2 / week to 5 times / 4 weeks; then - n / a / 10 million IU / m 2 /3 times a week / 48 weeks. Altevir is prescribed as an adjuvant therapy if there is a high risk of recurrence after tumor removal. Depending on the tolerability of the drug, dose adjustment is possible;
  • Multiple Myeloma: n / k / 3 million IU / m 2 / week 3 times / period achieve stable remission;
  • Kaposi's sarcoma against the background of AIDS: i / m or s / c / 10-12 million IU / m 2 per day (the optimal dose has not been determined) / when the disease stabilizes or responds to Altevir, therapy is carried out until it is necessary to cancel it or tumor regression will not occur;
  • Kidney cancer: n / k / 3-10 million IU / m 2 /3 times a week (Altevira optimal dosing regime is not installed).

When preparing a solution for intravenous administration, 100 ml of sterile 0.9% sodium chloride solution should be added to the required dose. The drug is administered for 20 minutes.

Side effects

  • Central nervous system: very often - headache; less often - insomnia, irritability, asthenia, suicidal attempts and thoughts, drowsiness, dizziness, depression; rarely - anxiety, nervousness;
  • Digestive system: very often - nausea, decreased appetite; less common - diarrhea, vomiting, change in taste, dry mouth; rarely - dyspepsia, abdominal pain; there may be a reversible increase in the activity of liver enzymes;
  • Musculoskeletal system: very often - myalgia; less often - arthralgia;
  • Cardiovascular system: often - lowering blood pressure; rarely - tachycardia;
  • Hematopoietic system: reversible granulocytopenia and leukopenia, thrombocytopenia, decreased hemoglobin levels;
  • General reactions: very often - weakness, fever (are reversible and dose-dependent reactions, disappear within 3 days after a break in therapy or its cancellation), chills; less often - malaise;
  • Dermatological reactions: less often - increased sweating, alopecia; rarely - itching and rash;
  • Others: rarely - autoimmune thyroiditis, weight loss.

special instructions

Prior to the appointment of Altevir in the treatment of chronic viral hepatitis B and C, a liver biopsy is recommended to assess the degree of liver damage (signs of fibrosis and / or an active inflammatory process). The effectiveness of therapy for chronic hepatitis C increases when combined with ribavirin. In cases of hepatic coma or decompensated liver cirrhosis, Altevir is ineffective.

With the development of adverse reactions, you need to reduce the dose of Altevir by 2 times or cancel the drug until they disappear. If the disturbances persist or reappear after a dose reduction, or if the disease progresses, Altevir is canceled.

With a decrease in the level of platelets below 50x10 9 / L or the level of granulocytes below 0.75x10 9 / L, it is recommended to halve the dose of Altevir under the control of a blood test after 7 days. If the described changes persist, the therapy is canceled.

In cases of a decrease in the level of platelets below 25x10 9 / l or the level of granulocytes below 0.5x10 9 / l, the drug is recommended to be canceled with a blood test control after 7 days.

When treated with interferon alpha-2b, antibodies can be detected in the blood serum of patients, which neutralize its antiviral activity. In most cases, antibody titers are low, their appearance does not lead to a decrease in the effectiveness of therapy or the development of other autoimmune disorders.

Drug interactions

The interaction of Altevir with other drugs / substances has not been fully studied.

Precautions require prescription in combination with sedative / hypnotics, narcotic analgesics, drugs that potentially have a myelodepressant effect.

With the combined appointment of Altevir with theophylline, it is necessary to control the serum concentration of the latter and, if necessary, adjust the doses used.

Altevir in combination with chemotherapeutic drugs (cytarabine, cyclophosphamide, doxorubicin, teniposide) increases the likelihood of toxic effects.
