Essential phospholipids are a hepatoprotector with clinically proven efficacy, which increases the liver's resistance to the pathological effects of the external and internal environment, enhances its detoxifying function and the ability to regenerate after various injuries.
Release form and composition
Essential phospholipids are available in the following forms:
- solution for intravenous administration: clear or almost transparent yellow liquid (5 ml each in ampoules made of neutral light-shielding glass, 5 ampoules in plastic blisters / pallets / blisters, in a cardboard box 1 or 2 plastic packaging / pallets / cell packaging, in a box (for hospitals), 50 or 100 plastic contour packs / pallets / blisters; in cardboard boxes with spacer cardboard inserts, 5 or 10 ampoules each);
- capsules [(biologically active additive (BAA)]: soft gelatinous, content weight 800 mg (60 pcs. in a package).
1 ml of solution contains:
- active ingredient: phospholipids (in terms of the basic substance - phosphatidylcholine) - 50 mg;
- auxiliary ingredients: benzyl alcohol, deoxycholic acid, sodium hydroxide, sodium chloride, sodium riboflavin phosphate (in terms of riboflavin), ethanol 96%, water for injection.

- active ingredient: essential phospholipids - 230 mg;

Pharmacological properties
Pharmacodynamics
The active component of the drug is essential phospholipids, which are the main elements of the structure of cell membranes and cell organelles. Liver diseases are always accompanied by their damage, leading to disruptions in the activity of enzymes and receptor systems associated with them, deterioration of liver function and a decrease in the ability of liver cells to regenerate.
The phospholipids included in the composition of the drug correspond in their chemical structure to endogenous phospholipids, but surpass them in activity due to the increased content of essential (polyunsaturated) fatty acids in them. The incorporation of such high-energy molecules into the destroyed areas of the cell membranes of hepatocytes regenerates the integrity of liver cells and promotes their restoration. Since double bonds of cis-isomers of polyunsaturated fatty acids prevent parallel arrangement of hydrocarbon chains in phospholipid membranes, the density of arrangement of phospholipid structures is weakened, thereby increasing the metabolic rate. The resulting functional blocks promote an increase in the activity of enzymes fixed on membranes, and normalize the physiological pathway of the most important metabolic processes.
Essential phospholipids are incorporated into disturbed lipid metabolism by regulating lipoprotein metabolism in such a way that cholesterol and neutral fats are transformed into transportable forms (mainly by increasing the ability of high density lipoproteins to bind cholesterol) and can be subject to oxidation. This mechanism has a normalizing effect on the following processes: detoxification function of the liver, metabolism of fats and proteins, regeneration and preservation of the structure of liver cells and phospholipid-dependent enzyme systems, which ultimately reduces the formation of connective tissue, reducing the risk of developing cirrhosis and liver fibrosis.
The excretion of phospholipids into bile stabilizes it and reduces the lithogenicity index.
Pharmacokinetics
Phosphatidylcholine, binding mainly with high density lipoproteins, enters, in particular, into the liver cells.
The half-life (T 1/2) of the choline component is 66 hours, of unsaturated fatty acids - 32 hours.
Indications for use
Solution of Essential Phospholipids is recommended for the following diseases / conditions:
- steatosis (fatty degeneration) of the liver, including in patients with diabetes mellitus;
- hepatitis (in acute and chronic forms), cirrhosis, hepatic precoma and coma, necrosis and / or toxic damage to liver cells;
- toxicosis of pregnant women;
- perioperative therapy, especially in the hepatobiliary system;
- radiation sickness;
- psoriasis (as an adjunct treatment).
BAA Essential phospholipids are taken if necessary:
- natural restoration of liver cells;
- protection of the liver from toxic damage and obesity;
- lowering cholesterol levels.
Contraindications
According to the instructions, Essential phospholipids in the form of a solution are contraindicated for women during breastfeeding, children under the age of 3 years and patients with hypersensitivity to any of its components.
The drug is used with caution in children over 3 years old and pregnant.
Capsules are contraindicated during pregnancy, during breastfeeding and in case of individual intolerance to their components.
Instructions for the use of Essential phospholipids: method and dosage
Solution
The solution is intended for intravenous administration; it is not administered intramuscularly or subcutaneously due to the high probability of local irritation reactions.
Recommended dosage in the absence of other doctor's recommendations: 1–2 ampoules (5–10 ml) per day by intravenous jet slow injection; in severe disease - 2–4 ampoules (10–20 ml) per day. For one injection, it is permissible to use up to 2 ampoules (10 ml) of the solution.
If possible, essential phospholipids are diluted with the patient's blood in a 1: 1 ratio. If it is not possible to use the patient's own blood, a 5% or 10% dextrose solution for infusion, which does not contain electrolytes, should be taken for dilution in a 1: 1 ratio.
The duration of the course of therapy is determined by the doctor individually, it can be either 2–5 or 10–20 days.
According to the doctor's prescription, the drug can be administered by drip, after diluting 1 ampoule in 250-300 ml of 5% dextrose solution, the infusion rate must be maintained in the range of 40-50 drops / min.
The solution must remain clear and free of foreign matter throughout the entire procedure. If possible, parenteral administration of essential phospholipids should be replaced with oral forms of phosphatidylcholine as soon as possible.
Capsules
Capsules Essential phospholipids are taken orally at the same time as food.
Recommended dosage for adult patients: 2 pcs. 3 times a day. The duration of the course is at least 30 days, if necessary, the course can be continued.
Side effects
The use of the solution can cause the following side reactions from systems and organs:
- digestive system: rarely - the use of high doses can cause diarrhea;
- immune system: rarely - due to the content of benzyl alcohol, hypersensitivity reactions may develop; very rarely - allergic skin reactions are possible (urticaria, rash, exanthema); with an unknown frequency - itching.
When taking capsules, side effects were not registered.
Overdose
There is no data.

When diluting the solution of Essential Phospholipids for intravenous administration, electrolyte preparations should not be used. Only clear solutions are required.

Application during pregnancy and lactation
Solution
- pregnancy: the drug contains benzyl alcohol, which is able to penetrate the fetal blood through the placental barrier (in newborns, both premature and born on time, the use of drugs containing benzyl alcohol is associated with the development of dyspnea with a fatal outcome), therefore, the use of Essential phospholipids during gestation is allowed only in case of a clinically significant excess of the expected benefit from therapy for the mother over the potential risk to the fetus;
- breastfeeding: there are no data on the safety of the drug, therapy is contraindicated.
Capsules
Prescribing capsules to pregnant and lactating women is contraindicated.
Pediatric use
Due to the presence of benzyl alcohol in the solution, the use of Essential Phospholipids for the treatment of children from birth to 3 years is contraindicated in pediatric practice. For children over 3 years old, the drug is prescribed with caution.
Capsules are a dietary supplement, their use in pediatric practice is contraindicated.
Drug interactions
There is no data on the pharmacological interaction of Essential Phospholipids with other substances / drugs.
It is forbidden to mix the solution of Essential phospholipids in one syringe with other drugs and to use solutions of sodium chloride 0.9% and Ringer for dilution.
