Tsiprolet is a fluoroquinolone, an antimicrobial drug.

Release form and composition

Dosage forms:

  • film-coated tablets: round biconvex, with a smooth surface, the shell and the core of the tablet is almost white or white (10 pcs. in a blister, in a cardboard box 1 or 2 blisters);
  • solution for infusion: colorless or light yellow transparent liquid (100 ml in a polyethylene bottle, 1 bottle in a cardboard box);
  • eye drops: transparent light yellow or colorless liquid (5 ml in a plastic dropper bottle, 1 bottle in a cardboard box).

1 tablet contains:

  • active substance: ciprofloxacin hydrochloride - 291.106 mg or 582.211 mg, which is equivalent to 250 mg or 500 mg of ciprofloxacin (respectively);
  • auxiliary components: croscarmellose sodium, corn starch, colloidal silicon dioxide, microcrystalline cellulose, magnesium stearate, talc;
  • shell composition: sorbic acid, hypromellose (6 cps), macrogol 6000, polysorbate 80, titanium dioxide, dimethicone, talc.

1 ml of solution contains:

  • active substance: ciprofloxacin - 2 mg;
  • auxiliary components: hydrochloric acid, sodium chloride, lactic acid, disodium edetate, sodium hydroxide, citric acid monohydrate, water for injection.

1 ml drops contain:

  • active substance: ciprofloxacin hydrochloride - 3.49 mg, which is equivalent to 3 mg of ciprofloxacin;
  • auxiliary components: disodium edetate, 50% solution of benzalkonium chloride, hydrochloric acid, sodium chloride, water for injection.

Indications for use

Film-coated tablets and solution for infusion

The use of Ciprolet in the form of tablets and solution is indicated for the treatment of infectious and inflammatory diseases caused by microorganisms sensitive to ciprofloxacin, including:

  • infections of the teeth, mouth, jaws, gastrointestinal tract;
  • ear, throat, and nose infections;
  • respiratory tract infections;
  • infections of the gallbladder and biliary tract;
  • kidney and urinary tract infections;
  • infections of the musculoskeletal system;
  • infections of mucous membranes, skin and soft tissues;
  • postpartum infections;
  • genital infections (prostatitis, gonorrhea, adnexitis);
  • sepsis;
  • peritonitis.

In addition, tablets and a solution are used as part of complex therapy with immunosuppressants for the prevention and treatment of infections in patients with reduced immunity.

Eye drops

The use of drops is indicated for the treatment of infectious and inflammatory pathologies of the eye and its appendages caused by bacteria that are sensitive to the drug:

  • subacute and acute conjunctivitis;
  • blepharoconjunctivitis, blepharitis;
  • corneal ulcers of bacterial etiology;
  • keratoconjunctivitis, bacterial keratitis;
  • chronic form of meibomitis and dacryocystitis;
  • infectious complications after surgery;
  • infectious complications after an eye injury or a foreign body entering it (including their prevention).

In addition, drops are used in ophthalmic surgery for preoperative prophylaxis.

Contraindications

  • the period of pregnancy and breastfeeding;
  • individual intolerance to drugs of the fluoroquinolone group;
  • hypersensitivity to the components of the drug.

With caution, Tsiprolet should be prescribed for cerebrovascular atherosclerosis, cerebrovascular accident, convulsive syndrome.

In addition, there are separate contraindications for each of the dosage forms.

Film-coated tablets and solution for infusion

  • pseudomembranous colitis;
  • age up to 18 years.

It should be used with caution in patients with mental disorders, epilepsy, severe renal and / or hepatic failure, in old age.

Eye drops

  • viral keratitis;
  • age up to 1 year.

Method of administration and dosage

Film-coated tablets

The tablets are taken orally, on an empty stomach (before meals), with a sufficient amount of liquid.

The doctor prescribes the dose and period of treatment individually based on clinical indications, taking into account the type of infection, concomitant pathologies, the severity of the condition, age and weight of the patient.

Recommended dosage regimen:

  • kidney and urinary tract infections: uncomplicated form - 250 mg, severe form - 500 mg, frequency of use - 2 times a day;
  • infections of the lower respiratory tract: medium severity - 250 mg, severe - 500 mg, frequency of application - 2 times a day;
  • gonorrhea: 250-500 mg once;
  • severe enteritis, colitis and gynecological infections (occurring with high fever), osteomyelitis, prostatitis: 500 mg 2 times a day;
  • usual diarrhea: 250 mg 2 times a day.

The duration of treatment is 7-10 days, after the complete disappearance of the symptoms of the disease, the tablets must be taken for several more days.

In case of severe renal dysfunction, the dose of the drug is reduced by 2 times.

In chronic renal failure, the dose depends on creatinine clearance (CC):

  • CC above 50 ml / min: usual dose;
  • CC 30-50 ml / min: 250-500 mg 1 time in 12 hours;
  • CC 5-29 ml / min: 250-500 mg 1 time in 18 hours;
  • with dialysis: 250-500 mg once every 24 hours after hemodialysis or peritoneal dialysis.

Solution for infusion

The solution is intended for intravenous (IV) drip administration.

The solution for infusion can be mixed with 0.9% sodium chloride solution, 10% fructose solution, 5% and 10% dextrose solution, Ringer's solution, a solution consisting of 5% dextrose solution and 0.225% or 0.45% sodium chloride solution.

When prescribing a dose, clinical indications, type of infection, condition, age and weight of the patient, and comorbidities should be taken into account.

The duration of the infusion should be at the rate of 0.5 hour for the introduction of 200 mg of the drug.

Recommended dosage: moderate infections - 200 mg 2 times per knock, severe - 400 mg 2 times a day. The duration of therapy is 7-14 days or more.

In acute gonorrhea, the patient is prescribed 100 mg once.

Prevention of postoperative infections is carried out by administering 200-400 mg of the drug 0.5-1 hour before the operation.

Eye drops

Drops are intended for topical application by instillation into the conjunctival sac of the eye affected by the infection.

Recommended dosage:

  • mild to moderately severe infection: 1-2 drops every 4 hours;
  • severe form of infection: 2 drops every hour, if the condition improves, the dose and frequency of instillations should be reduced;
  • bacterial corneal ulcer: 1 drop according to the scheme - on the first day of therapy for six hours with an interval of 15 minutes, then only during wakefulness - every 0.5 hours; on the second day of treatment - every hour during the waking period; from 3 to 14 days - during wakefulness every 4 hours. In the absence of epithelialization after 14 days of therapy, the use of the drug can be continued.

Side effects

Film-coated tablets and solution for infusion

  • nervous system: headache, dizziness, increased fatigue, tremors, insomnia, anxiety, nightmares, peripheral paralgesia, sweating, increased intracranial pressure, confusion, hallucinations, depression, other psychotic reactions (sometimes with the risk of self-harm by the patient), fainting, migraine, cerebral artery thrombosis;
  • digestive system: abdominal pain, nausea, vomiting, diarrhea, flatulence, anorexia, hepatitis, cholestatic jaundice (more often with a history of liver disease), hepatonecrosis, increased activity of liver enzymes and alkaline phosphatase;
  • sense organs: impairment of smell and taste, change in color perception, diplopia, hearing loss, tinnitus;
  • hematopoietic system: anemia, leukopenia, granulocytopenia, thrombocytopenia, leukocytosis, hemolytic anemia, thrombocytosis;
  • cardiovascular system: lowering blood pressure (BP), heart rhythm disturbances, tachycardia, flushing of the skin of the face;
  • musculoskeletal system: arthritis, arthralgia, tendovaginitis, myalgia, tendon ruptures;
  • urinary system: urinary retention, hematuria, crystalluria (more often with low urine output and alkaline urine), glomerulonephritis, albuminuria, hematuria, dysuria, urethral bleeding, decreased renal nitrogen excretion function, polyuria, interstitial nephritis;
  • laboratory parameters: hypercreatininemia, hyperbilirubinemia, hypoprothrombinemia, hyperglycemia;
  • allergic reactions: itching, shortness of breath, urticaria, edema of the face or larynx, drug fever, eosinophilia, petechiae, vasculitis, increased photosensitivity, erythema nodosum, Stevens-Johnson syndrome, exudative erythema multiforme, Lyell's syndrome, blistering accompanied by bleeding nodules forming scabs;
  • others: superinfection (pseudomembranous colitis, candidiasis), general weakness.

In addition, against the background of the application of the solution, local reactions may occur in the form of pain and burning at the injection site, phlebitis.

Eye drops

  • organ of vision: mild pain, conjunctival hyperemia, itching, burning; rarely - photophobia, lacrimation, eyelid edema, sensation of a foreign body in the eyes, decreased visual acuity, corneal infiltration, keratitis, keratopathy, in patients with corneal ulcers - the appearance of a white crystalline precipitate;
  • others: nausea, allergic reactions; rarely - the development of superinfection, a feeling of an unpleasant taste in the mouth immediately after instillation.

special instructions

Due to the risk of side effects from the central nervous system, patients with a history of seizures, epilepsy, organic brain lesions, and vascular pathologies can be prescribed Tsiprolet exclusively for health reasons.

If severe and prolonged diarrhea appears on the background of therapy, it is necessary to exclude the presence of pseudomembranous colitis; if the diagnosis is confirmed, immediate withdrawal of the tablets and solution is required.

Due to the possible inflammation of the tendons or their rupture against the background of the use of tablets and a solution of the drug, treatment should be discontinued at the first signs of tendovaginitis or pain in the tendons.

Oral and parenteral administration of the drug should be accompanied by the intake of a sufficient amount of fluid in patients with normal diuresis.

It is recommended to avoid direct sunlight during the treatment period.

Do not wear contact lenses when using drops.

Eye drops should not be injected into the anterior chamber of the eye or subconjunctivally.

With the simultaneous use of other ophthalmic solutions, the interval between procedures should be 5 minutes or more.

The use of Tsiprolet has a negative effect on the patient's ability to drive vehicles and mechanisms.

Drug interactions

With the simultaneous use of Tsiprolet:

  • didanosine reduces the absorption of ciprofloxacin;
  • theophylline can increase the concentration in blood plasma and the risk of developing its toxic effect;
  • medicines containing ions of aluminum, zinc, iron or magnesium, and antacids can reduce the absorption of ciprofloxacin, therefore, the interval between taking these funds should be at least 4 hours;
  • cyclosporine enhances its nephrotoxic effect;
  • other antimicrobial drugs (aminoglycosides, beta-lactams, clindamycin, metronidazole) cause a synergistic effect;
  • non-steroidal anti-inflammatory drugs (except acetylsalicylic acid) increase the likelihood of seizures;
  • metoclopramide accelerates the absorption of ciprofloxacin;
  • uricosuric agents increase the plasma concentration of ciprofloxacin;
  • indirect anticoagulants enhance their effect, lengthen the bleeding time.

Recommended combinations for the treatment of specific diseases:

  • infections caused by Pseudomonas spp: azlocillin, ceftazidime;
  • streptococcal infections: mezlocillin, azlocillin and other beta-lactam antibiotics;
  • staphylococcal infections: isoxazolylpenicillins, vancomycin;
  • anaerobic infections: metronidazole, clindamycin.

The acidity (pH) of the ciprofloxacin infusion solution is 3.5-4.6, therefore it is pharmaceutically incompatible with infusion solutions and preparations unstable in an acidic environment. For intravenous administration, it is impossible to mix with solutions with a pH of more than 7.
