Release form and composition
Dosage form - gel for local and external use, light yellow or colorless, with a specific odor (in a cardboard box 1 tube containing 15 or 30 g of gel, and instructions for the use of Trombogel 1000).
Composition of 1 g of gel:
- active substance: sodium heparin - 1000 international units (ME);
- auxiliary components: purified water - up to 1 g; triethanolamine (trolamine) - to obtain a pH in the range from 5.5 to 7.5; ethanol - 100 mg; propyl parahydroxybenzoate - 0.5 mg; methyl parahydroxybenzoate - 1.5 mg; levomenthol - 0.5 mg; carbopol 980 NF (carbomer) - 11 mg.

Sodium heparin is one of the direct-acting anticoagulants (a group of medium molecular weight heparins). It is characterized by local antithrombotic, antiexudative and moderately anti-inflammatory activity when applied externally.
After applying the gel, the fibrinolytic properties of the blood are activated, the action of hyaluronidase is inhibited and the formation of thrombin stops. The active substance of the drug, gradually released and passing through the skin, reduces the inflammatory process, exhibits antithrombotic properties, increases microcirculation and activates tissue metabolism, which leads to a decrease in tissue swelling, acceleration of the resorption of blood clots and hematomas.
Pharmacokinetics
Heparin sodium is characterized by low absorption when applied externally.
Indications for use
- thrombophlebitis of superficial veins (prevention and therapy);
- hemorrhoids, including postpartum;
- superficial mastitis;
- post-infusion and post-injection phlebitis;
- lymphangitis;
- elephantiasis;
- localized infiltrates and edema;
- superficial periphlebitis;
- subcutaneous hematoma;
- bruises and injuries, including joints, tendons, muscle tissue.
Contraindications
Absolute:
- traumatic violation of the integrity of the skin;
- ulcerative necrotic processes;
- established hypersensitivity to the components of the drug.
Relative (the use of Trombogel 1000 requires special care and careful medical supervision):
- thrombocytopenia;
- increased tendency to bleeding.
Trombogel 1000, instructions for use: method and dosage
Gel Trombogel 1000 is intended for local and external use.
When applied externally, a single dose of the drug (a column with a length of 3 to 10 cm - 0.5-1 g) is applied to the affected area of the skin (with a diameter of 3-5 cm) from 1 to 3 times a day.
For the treatment of thrombosis of hemorrhoidal veins, the drug is applied to coarse calico pads, which are then applied directly to the restrained nodes and fixed with a bandage. A swab soaked in the drug can also be inserted rectally. The duration of treatment is 3-4 days.
In case of a leg ulcer, the inflamed skin around the ulcer is lubricated with gel 2-3 times daily, until the inflammatory phenomena disappear - on average, from 3 to 7 days. A longer course of therapy is allowed as directed by a doctor.
Side effects
During therapy with Thrombogel 1000, the development of undesirable phenomena in the form of allergic reactions and skin flushing is possible.
Overdose
Since the absorption of sodium heparin is low, the development of an overdose is unlikely. Such cases have not been described.
With prolonged use of the gel, the development of symptoms of intoxication in the form of hemorrhagic complications is possible.
In this case, it is recommended to cancel the drug; if necessary, the appointment of protamine sulfate (heparin antagonist).
special instructions
Gel Trombogel 1000 cannot be used in the presence of purulent processes, applied to mucous membranes and open wounds. The drug is not recommended for patients with deep venous thrombosis.
Application during pregnancy and lactation
There are no data regarding the use of Trombogel 1000 during pregnancy and during breastfeeding.
Drug interactions
Heparin sodium should not be used topically in combination with tetracyclines, antihistamines and non-steroidal anti-inflammatory drugs.
