  • active substance: trimetazidine dihydrochloride - 20 mg;

Pharmacological properties

Pharmacodynamics

Trimectal is an antianginal and antihypoxic agent. The active substance of the drug, having a direct effect on the neurons of the brain and cardiomyocytes, ensures their optimal functioning and improves metabolism. The cytoprotective effect demonstrated by trimetazidine is due to an increase in the energy potential, an increase in the activity of the oxidative decarboxylation process, and an improvement in oxygen consumption (by blocking fatty acid oxidation and enhancing aerobic glycolysis).

Providing normal myocardial contractility, the agent counteracts the decrease in the intracellular volume of ATP and phosphocreatine. Against the background of acidosis, due to the effect of trimetazidine, the functions of membrane ion channels and the intracellular level of potassium ions are normalized, and there is no accumulation of calcium and sodium in cardiomyocytes. The drug reduces the concentration of phosphates and intracellular acidosis associated with ischemia of the heart muscle and reperfusion. By preventing the negative action of free radicals, it helps to maintain the integrity of cell membranes.

Trimectal does not allow activation of neutrophils in the ischemic zone, promotes an increase in the duration of the electrical potential, a decrease in the severity of ischemic damage to the myocardium and the release of creatine phosphokinase from cells. Leading to a decrease in the frequency of angina attacks, the drug reduces the need for taking nitrates, 14 days after the start of the course, it reduces fluctuations in blood pressure (BP) and increases exercise tolerance.

Against the background of trimetazidine therapy, there is an improvement in the results of vestibular tests and hearing acuity, a decrease in tinnitus and dizziness. With vascular diseases of the eyes, the functional activity of the retina is restored.

Pharmacokinetics

After oral administration, trimetazidine is rapidly and almost completely absorbed from the gastrointestinal tract (GIT). Bioavailability is 90%, the period of reaching the maximum concentration of the agent (C max) in blood plasma is 2 hours. With a single dose of 20 mg of the drug, C max can be approximately equal to 55 ng / ml. The agent passes easily through the histohematogenous barriers.

The binding of trimetazidine to plasma proteins is no more than 16%, the half-life (T ½) is 4.5–5 hours. It is excreted by the kidneys, unchanged - about 60%.

Indications for use

  • ischemic heart disease (CHD) - prevention of angina attacks (as part of combination therapy);
  • dizziness of vascular origin;
  • chorioretinal vascular disorders;
  • cochleo-vestibular disorders of the ischemic nature (hearing impairment, tinnitus).

Contraindications

  • severe functional disorders of the liver;
  • renal failure;
  • age up to 18 years;
  • pregnancy and the period of breastfeeding;
  • hypersensitivity to the constituents of Trimectal.

Instructions for use of Trimectal: method and dosage

Trimectal is taken orally with meals.

The recommended daily dose is 40-60 mg (2-3 capsules), divided into 2-3 doses. The duration of drug use is determined by the attending physician individually.

Side effects
