Thioctacid BV is a metabolic drug with an antioxidant effect.

Release form and composition

Thioctacid BV is produced in the form of film-coated tablets: green-yellow, oblong biconvex (30, 60 or 100 pieces in dark glass bottles, 1 bottle in a cardboard box).

1 tablet contains:

  • active substance: thioctic (alpha-lipoic) acid - 0.6 g;
  • auxiliary components: magnesium stearate, hyprolose, low-substituted hyprolose;
  • composition of the film shell: titanium dioxide, macrogol 6000, hypromellose, aluminum varnish based on indigo carmine and quinoline yellow dye, talc.

Thioctacid BV is a metabolic drug that improves the trophism of neurons, has hepatoprotective, hypocholesterolemic, hypoglycemic, hypolipidemic effects.

The active ingredient of the drug is thioctic acid, which is found in the human body and is an endogenous antioxidant. As a coenzyme, it participates in the reactions of oxidative phosphorylation of pyruvic acid and alpha-keto acids. The mechanism of action of thioctic acid is close to the biochemical effect of vitamins of group B. It helps to protect cells from the toxic effects of free radicals that arise in metabolic processes, and neutralizes exogenous toxic compounds that have entered the body. By increasing the level of the endogenous antioxidant glutathione, it causes a decrease in the severity of the symptoms of polyneuropathy.

The result of the synergistic action of thioctic acid and insulin is an increase in glucose utilization.

Pharmacokinetics

Absorption of thioctic acid from the gastrointestinal tract (GIT) when taken orally occurs quickly and completely. Taking the drug with meals can reduce its absorption. C max (maximum concentration) in blood plasma after taking a single dose is reached after 30 minutes and is 0.004 mg / ml. The absolute bioavailability of Thioctacid BV is 20%.

Before entering the systemic circulation, thioctic acid undergoes the effect of the first passage through the liver. The main pathways of its metabolism are oxidation and conjugation.

According to the instructions, Thioctacid BV 600 mg is taken orally on an empty stomach, 0.5 hours before breakfast, swallowing whole and drinking plenty of water.

Recommended dosage: 1 pc. 1 per day.

Given the clinical feasibility, for the treatment of severe forms of polyneuropathy, it is possible to initially prescribe a solution of thioctic acid for intravenous administration (Thioctacid 600 T) for a period of 14 to 28 days, followed by the transfer of the patient to a daily intake of the drug inside (Thioctacid BV).

Side effects

  • from the digestive system: often - nausea; very rarely - vomiting, pain in the stomach and intestines, diarrhea, impaired taste;
  • from the nervous system: often - dizziness;
  • allergic reactions: very rarely - itching, skin rash, urticaria, anaphylactic shock;
  • on the part of the body as a whole: very rarely - a decrease in blood glucose levels, the appearance of symptoms of hypoglycemia in the form of headache, confusion, increased sweating, visual impairment.

Overdose

Symptoms: against the background of a single dose of 10–40 g of thioctic acid, severe intoxication may develop with such manifestations as generalized convulsive seizures, hypoglycemic coma, severe acid-base imbalance, lactic acidosis, severe blood clotting disorders (including fatal).

Treatment: if an overdose of Thioctacid BV is suspected (a single dose for an adult more than 10 tablets, a child - more than 50 mg per 1 kg of his body weight), the patient needs immediate hospitalization with the appointment of symptomatic therapy. If necessary, use anticonvulsant therapy, urgent measures aimed at maintaining the functions of vital organs.

Since ethanol is a risk factor for the development of polyneuropathy and causes a decrease in the therapeutic efficacy of Thioctacid BV, alcohol consumption is categorically contraindicated in patients.

Application during pregnancy and lactation

Pediatric use

Drug interactions

With the simultaneous use of Thioctacid BV:

  • cisplatin - lowers its therapeutic effect;
  • insulin, oral hypoglycemic agents - can enhance their effect, therefore, regular monitoring of blood glucose levels is required, especially at the beginning of combination therapy; if necessary, a decrease in the dose of hypoglycemic drugs is allowed;
  • ethanol and its metabolites - cause weakening of the drug.

It is necessary to take into account the property of thioctic acid to bind metals when combined with drugs containing iron, magnesium and other metals. It is recommended to postpone their reception to the second half of the day.
