Sustak forte extended-release tablets (6.4 mg)
Sustak forte extended-release tablets (6.4 mg)

Sustak forte is a drug with vasodilating, antianginal, hypotensive and vasodilating action.

Release form and composition

Dosage form - tablets of prolonged action (25 pcs. In vials, plastic cases and polystyrene tubes, in a cardboard box 1 bottle, or 1 case, or 1 tube).

Active ingredient: nitroglycerin, in 1 tablet - 6.4 mg.

Indications for use

Sustak forte is intended for the prevention and relief of angina attacks.

Also, the drug is used for rehabilitation treatment after myocardial infarction.

Contraindications

  • arterial hypotension (systolic blood pressure <100 mm Hg, diastolic - <60 mm Hg);
  • constrictive pericarditis;
  • hypertrophic obstructive cardiomyopathy;
  • acute myocardial infarction with severe arterial hypotension;
  • cardiac tamponade;
  • aortic stenosis;
  • angle-closure glaucoma with high intraocular pressure;
  • increased intracranial pressure (including with hemorrhagic stroke, after a recent head injury);
  • cerebral hemorrhage;
  • collapse;
  • shock;
  • toxic pulmonary edema;
  • hypersensitivity to the drug or other nitrates.

Carefully:

  • elderly age;
  • severe cerebral atherosclerosis;
  • tendency to orthostatic hypotension;
  • disorders of cerebral circulation;
  • severe anemia.

Method of administration and dosage

Sustak forte should be taken orally, swallowing the tablets whole, without chewing.

The doctor selects the dose in each case individually.

The daily dose, depending on the severity of the disease, varies from 20 to 80 mg, it must be divided into 3-4 doses.

Side effects

  • from the central nervous system: dizziness, headache; rarely (especially in case of overdose) - anxiety, psychotic reactions;
  • on the part of the cardiovascular system: a feeling of heat, hyperemia of the skin, arterial hypotension, tachycardia; rarely (especially in case of overdose) - cyanosis, collapse;
  • from the digestive system: nausea, vomiting;
  • allergic reactions: rarely - itching, rash;
  • others: methemoglobinemia.

special instructions

With prolonged treatment, the likelihood of developing tolerance to the action of nitrates appears. To avoid such a reaction, it is recommended to observe a 10-12 hour break in taking the drug during each 24-hour cycle.

During treatment, alcohol is prohibited.

Sustak forte can affect the speed of psychophysical reactions. This should be taken into account by vehicle drivers and people employed in potentially hazardous industries.

Drug interactions

  • angiotensin-converting enzyme inhibitors, vasodilators, calcium channel blockers, diuretics, beta-blockers, monoamine oxidase inhibitors, tricyclic antidepressants, ethanol and ethanol-containing agents: the hypotensive effect of nitroglycerin is enhanced;
  • calcium channel blockers, beta-blockers: antianginal effect is enhanced;
  • sympathomimetics: the antianginal effect of nitroglycerin may decrease, in turn nitroglycerin may reduce the pressor effect of sympathomimetics, as a result of which arterial hypotension is possible;
  • drugs with anticholinergic activity (including disopyramide, tricyclic antidepressants): dry mouth appears, hyposalivation develops;
  • norepinephrine, acetylcholine, histamine: their effect decreases;
  • dihydroergotamine: its bioavailability increases, the antianginal effect of nitroglycerin decreases;
  • novocainamide: there is a risk of increased hypotensive effect, development of collapse;
  • sumatriptan, rizatriptan: the risk of coronary artery spasm increases;
  • quinidine: orthostatic collapse is possible;
  • sildenafil: there is a possibility of severe arterial hypotension and myocardial infarction;
  • ethanol: severe weakness develops, dizziness appears.

A number of studies have noted a decrease in the vasodilating effect of nitroglycerin in patients receiving long-term acetylsalicylic acid (ASA).

There is limited evidence that ASA, used as an analgesic, increases the plasma concentration of nitroglycerin, which may be accompanied by headaches and an increase in the hypotensive effect.

Nitroglycerin, presumably, can enhance the antiplatelet effect of ASA.
