Droperidol is a drug with antiemetic, neuroleptic and anti-shock effects.

Release form and composition

Droperidol is produced in the form of a solution for injection (in ampoules of 2 or 5 ml, 5 or 10 ampoules in a carton box, or 5 ampoules in blisters, 1, 2, 20, 30, 50, 100 packs in a carton box; 5 or 10 ml in vials, 5 vials in blisters, 30 or 50 packages in a cardboard box).

The active substance is a part of 1 ml of solution: droperidol - 2.5 mg.

Indications for use

  • Surgery: induction anesthesia, premedication, potentiation of general and regional anesthesia; neuroleptanalgesia (along with fentanyl); providing a sedative effect, eliminating vomiting and pain in the postoperative period, vomiting and nausea during surgical and diagnostic procedures;
  • Therapy: shock and pain in trauma, severe attacks of angina pectoris, myocardial infarction, hypertensive crisis, pulmonary edema;
  • Psychiatric practice: hallucinations, psychomotor agitation.

Contraindications

  • Coma;
  • Extrapyramidal disorders;
  • Increased QT interval on ECG;

The use of Droperidol by pregnant women is possible only in cases where the intended benefits to the health of the mother are higher than the potential risk to the fetus. If it is necessary to use the drug by women during lactation, breastfeeding should be discontinued.

Method of administration and dosage

The dose of Droperidol is determined individually, taking into account the nature of the disease, age, general physical condition, body weight, drugs used simultaneously, and the type of anesthesia to be taken.

For premedication, adults are injected intramuscularly with 2.5-5 mg of droperidol 15-45 minutes before the start of the surgical intervention, for children - at the rate of 100 μg / kg.

For adults, for introduction into anesthesia, the drug is prescribed in a dose of 15-20 mg (intravenously). For children, intravenous (at a dose of 200-400 μg / kg) or intramuscular (at a dose of 300-600 μg / kg) administration is possible.

To maintain anesthesia during long-term operations, repeated intravenous administration of Droperidol at a dose of 2.5-5 mg is possible.

In the postoperative period, adults are prescribed intramuscularly 2.5-5 mg every 6 hours.

Side effects

When using Droperidol, it is possible to develop disorders on the part of some body systems:

  • Central nervous system: drowsiness, dysphoria in the postoperative period and, on the contrary, with the use of higher doses - fear, motor excitability, anxiety; rarely - extrapyramidal symptoms; in some cases in the postoperative period - depression, hallucinations;
  • Cardiovascular system: tachycardia and moderate arterial hypotension (usually no special therapy is required); in very rare cases - arterial hypertension (most likely - when combined with fentanyl or other parenterally administered analgesics);
  • Digestive system: dyspeptic symptoms, loss of appetite, nausea; rarely - transient liver dysfunctions, jaundice;
  • Allergic reactions: rarely - dizziness, anaphylactic reactions, tremors, bronchospasm, laryngospasm.

Droperidol should be used only in a hospital setting.

The drug should be prescribed with caution in patients with functional disorders of the kidneys and liver, depression, epilepsy, as well as in conditions preceding an epileptic seizure.

With pheochromocytoma, after the administration of Droperidol, tachycardia and severe arterial hypertension may develop.

During therapy, it is necessary to anticipate the development of severe arterial hypotension. Also, the drug can cause a decrease in pressure in the pulmonary artery, which must be taken into account during diagnostic and surgical procedures. Patients receiving droperidol need careful medical supervision.

The initial dose of the drug should be reduced in elderly patients, exhausted and physically weakened patients. When increasing the dose, you need to be guided by the effect obtained.

Droperidol should be prescribed in a lower dose when used simultaneously with drugs that have a depressing effect on the central nervous system. Accordingly, after Droperidol, the dose of such drugs is also reduced.

The use of Droperidol in high doses (25 mg or more) can lead to sudden death in patients with cardiac arrhythmias on the background of alcohol withdrawal, hypoxia or electrolyte imbalance.

In surgical practice, when using the drug, it is necessary to carefully monitor the parameters of the physiological state of the body. When conducting epidural or spinal anesthesia, it is possible to develop a blockade of the sympathetic nervous system and intercostal nerves, which, in turn, can lead to difficulty breathing, the development of arterial hypotension and the expansion of peripheral vessels.

To avoid the occurrence of orthostatic hypotension, it is recommended to be careful when transporting the patient, to avoid sudden changes in body position.

Against the background of the action of Droperidol and within 24 hours after its use, it is necessary to avoid performing potentially hazardous work that requires quick psychomotor reactions and high concentration of attention.

Drug interactions

With the simultaneous use of Droperidol with certain drugs, undesirable effects may occur:

  • Drugs that have a depressing effect on the central nervous system (benzodiazepine derivatives, drugs for anesthesia, opioid analgesics, hypnotics): increasing the depressing effect on the central nervous system;
  • Antihypertensive drugs: potentiation of their action;
  • Epinephrine and other adreno- and sympathomimetic agents: manifestation of antagonism in their relation;
  • Dopamine agonists, including bromocriptine, lisuride and levodopa: inhibiting their action.
