Dapril is an angiotensin-converting enzyme (ACE) inhibitor.

Release form and composition

Dapril is available in the form of tablets (10 pcs. In a blister strip, in a cardboard box: 5 mg and 10 mg - 3 packs, 20 mg - 2 packs).

1 tablet contains:

  • active substance: lisinopril - 5 mg, 10 mg or 20 mg;
  • auxiliary components: calcium hydrogen phosphate, mannitol, iron oxide (E172), magnesium stearate, gelatinized starch, starch.

Indications for use

  • arterial hypertension, including renovascular hypertension - monotherapy or as part of combination therapy with other antihypertensive drugs;
  • chronic heart failure - in combination with diuretics and / or digitalis drugs.

Contraindications

  • a history of angioedema;
  • primary hyperaldosteronism;
  • severe renal dysfunction;
  • bilateral stenosis of the renal arteries or stenosis of an artery of a solitary kidney with progressive azotemia;
  • azotemia;
  • condition after kidney transplantation;
  • hyperkalemia;
  • aortic stenosis and similar hemodynamic disorders;
  • childhood;
  • II and III trimesters of pregnancy;
  • breast-feeding;
  • hypersensitivity to ACE inhibitors and drug components.

Method of administration and dosage

The tablets are taken orally.

The doctor prescribes the dose of the drug individually based on clinical indications and individual needs to achieve a sustainable effect.

Recommended dosage:

  • arterial hypertension: initial dose - 10 mg once a day. Further, the dose is selected individually, taking into account the patient's blood pressure (BP) level, the usual maintenance dose is 20 mg once a day, in the absence of a sufficient therapeutic effect after 7 days of therapy, it can be increased to 40 mg. The maximum daily dose is 80 mg;
  • chronic heart failure: the initial dose is 2.5 mg per day, the maintenance dose is 5–20 mg per day.

In case of impaired renal function, the daily dose is set taking into account creatinine clearance (CC):

  • CC more than 30 ml / min: 10 mg;
  • CC 10-30 ml / min: 5 mg;
  • CC less than 10 ml / min: 2.5 mg.

Side effects

  • on the part of the cardiovascular system: rarely - tachycardia, orthostatic hypotension;
  • from the nervous system: feeling tired, headache, dizziness; sometimes - confusion, instability of mood;
  • on the part of the hematopoietic system: agranulocytosis, neutropenia, a decrease in hemoglobin levels, a decrease in the number of erythrocytes;
  • from the digestive system: nausea; rarely - dry mouth, abdominal pain, diarrhea; sometimes - an increase in the activity of liver enzymes, an increase in the level of bilirubin in the blood serum;
  • allergic reactions: rarely - skin rash; sometimes - Quincke's edema;
  • from the respiratory system: dry cough;
  • others: sometimes - hyperkalemia, renal dysfunction.

special instructions

The use of ACE inhibitors can cause a side effect in the form of a dry cough, which disappears after the drug is discontinued. This should be taken into account in the differential diagnosis of cough in a patient taking Dapril.

The cause of a pronounced decrease in blood pressure is a decrease in the body's fluid volume caused by diarrhea or vomiting, concomitant use of diuretics, reduced salt intake, or dialysis. Therefore, it is recommended to start treatment under the strict supervision of a physician and to increase the dose of the drug with caution.

When carrying out hemodialysis using membranes with high permeability, there is a high risk of developing an anaphylactic reaction. Therefore, for dialysis, it is necessary to use only a different type of membrane or replace the drug with another antihypertensive agent.

Drug interactions

With the simultaneous use of Dapril:

  • potassium-sparing diuretics (triamterene, spironolactone, amiloride), potassium-containing agents containing potassium salt substitutes - increase the risk of hyperkalemia, especially with impaired renal function;
  • diuretics, antidepressants - cause a marked decrease in blood pressure;
  • non-steroidal anti-inflammatory drugs - reduce the antihypertensive effect of the drug;
  • lithium preparations - slow down the rate of their elimination from the body;
  • ethanol - enhances the effect of the drug.
