Pharmacological properties
Pharmacodynamics
Betahistine acts primarily on histamine H 1 and H 3 receptors localized in the vestibular nuclei of the central nervous system (CNS) and the inner ear. The drug helps to improve the permeability of the capillaries of the inner ear and microcirculation processes in them, and also normalizes the pressure of endolymph in the cochlea and labyrinth through a direct agonistic effect on the H 1 receptors located in the vessels of the inner ear, as well as an indirect effect on the H 3 receptors. At the same time, the active ingredient of the drug increases blood flow in the basilar artery.
Betahistine is characterized by a pronounced central effect, being an inhibitor of H 3 receptors, the localization of which is the nuclei of the vestibular nerve. The drug improves the conductivity in the neurons of the vestibular nuclei at the level of the brain stem. The above properties provide a therapeutic effect, which is expressed in the normalization of hearing in case of deterioration, a decrease in tinnitus, a decrease in the intensity and frequency of dizziness. Betahistine enhances the tone of the gastrointestinal tract (GIT) and bronchial smooth muscles, and can also activate the secretion of gastric juice.
Pharmacokinetics
Betahistine is rapidly absorbed from the gastrointestinal tract and slightly binds to blood plasma proteins. Its maximum content in blood plasma is determined 3 hours after ingestion. The half-life is 3-4 hours. Betahistine is almost 100% excreted in the urine as a metabolite (2-peridylacetic acid) for 1 day.
