Atifin is a drug with a wide spectrum of antifungal action. Belongs to the group of allylamines.

Release form and composition

Atifin is available in the following forms:

  • tablets: biconvex, round, white or almost white, with a chamfer and a notch on one of the sides (7 pieces in blisters, in a cardboard box 2 or 4 blisters);
  • cream for external use 1%: homogeneous, white or almost white (15 g each in aluminum tubes, one tube in a cardboard box).

Composition of 1 tablet:

  • active substance: terbinafine (in the form of terbinafine hydrochloride) - 125 mg or 250 mg;
  • auxiliary components: croscarmellose sodium, talc, magnesium stearate, microcrystalline cellulose, colloidal anhydrous silicon, lactose monohydrate, povidone.

Composition of 1 g of cream:

  • active substance: terbinafine (in the form of terbinafine hydrochloride) - 10 mg;
  • auxiliary components: sorbitan stearate, cetyl alcohol, polysorbate 60, sodium hydroxide, isopropyl myristate, benzyl alcohol, cetyl palmitate, stearyl alcohol, distilled water.

Indications for use

Pills

  • candidiasis of the mucous membranes and skin;
  • mycoses of nails and skin (microsporia, trichophytosis, epidermophytosis);
  • mycoses of the scalp (microsporia, trichophytosis);
  • dermatomycosis of smooth skin of the limbs and trunk, requiring systemic treatment;
  • onychomycosis.

Cream for external use

  • yeast infections of the skin (mainly those caused by the fungus of the genus Candida, in particular diaper rash);
  • treatment and prevention of fungal infections of the skin (inguinal epidermophytosis, dermatomycosis of the feet, fungal lesions of smooth skin of the body caused by dermatophytes, including Trichophyton mentagrophytes, Trichophyton violaceum, Trichophyton rubrum, Trichophyton verrucosum, Epidermophyton floccosum and Microsporum);
  • versicolor versicolor, caused by Pityrosporum orbiculare.

Contraindications

Absolute:

  • the period of pregnancy and lactation (for tablets);
  • children under two years of age, since there is insufficient data on the use of the drug in children weighing less than 12 kg (for tablets);
  • hypersensitivity to the main or auxiliary components of the drug.

Relative (Atifin is used with caution):

Method of administration and dosage

Pills

Atifin in tablet form is taken orally after meals. The dosage regimen and duration of therapy are set individually depending on the severity of the disease and the localization of the process.

Adult patients are usually prescribed 1 tablet (250 mg) once a day.

Recommended doses for children:

  • children weighing from 12 to 20 kg - 62.5 mg once a day;
  • children weighing from 20 to 40 kg - 125 mg once a day;
  • children weighing more than 40 kg - 250 mg once a day.

Recommended duration of treatment for various diseases:

  • trichophytosis of the feet - from 2 to 6 weeks;
  • candidiasis of the skin - from 2 to 4 weeks;
  • epidermophytosis inguinal and smooth skin trichophytosis - from 2 to 4 weeks;
  • ringworm (occurs mainly in children) - 4 weeks;
  • Onychomycosis - 6 (finger infections) to 12 (toe infections) weeks in some cases, for infections of the toes, the course of treatment is six months or more.

In severe renal and / or liver dysfunction, the dose of Atifin is 125 mg once a day.

Cream for external use

Atifin in the form of a cream for external use is used in adult patients and children over 12 years old. The drug is applied to the affected skin, as well as adjacent areas 1-2 times a day. The cream is applied in a thin layer, rubbing lightly. Before using Atifin, the affected areas are thoroughly cleaned and dried.

For infections that are accompanied by diaper rash (between the buttocks, under the mammary glands, in the groin area, in the spaces between the fingers), the application site can be covered with a gauze bandage, especially at night.

Recommended doses and duration of treatment:

  • candidiasis of the skin: 1 time per day for one week;
  • inguinal dermatomycosis and dermatomycosis of smooth skin: 2 times a day (morning and evening) for one week;
  • dermatomycosis of the feet: 1 time per day for two weeks with dermatomycosis of the upper and lateral parts of the foot and 2 times a day (morning and evening) for one week with dermatomycosis between the toes;
  • tinea versicolor: 1 time per day for two weeks.

In the absence of improvement after one to two weeks of therapy, the diagnosis should be clarified: to determine the causative agent of the disease, as well as its sensitivity to the drug, and, if necessary, adjust the treatment.

Elderly patients are prescribed Atifin in standard doses, no dosage adjustment is required.

Side effects

Systemic side effects that can occur when taking Atifin tablets are usually mild to moderate and temporary. Undesirable side effects are possible from the following systems and organs:

  • digestive system: often - loss of appetite, feeling of a full stomach, nausea, diarrhea, dyspepsia, abdominal pain; sometimes - taste disturbance; rarely - hepatitis, cholestatic jaundice, asymptomatic elevation of hepatic transaminases;
  • central and peripheral nervous system: rarely - decreased sensitivity to external stimuli, dizziness, paresthesia; very rarely - increased anxiety, depression;
  • hematopoietic system: very rarely - agranulocytosis, neutropenia, thrombocytopenia;

  • allergic reactions: often - erythema multiforme, urticaria; rarely - angioedema and reactions resembling serum sickness; very rarely - photosensitivity, Stevens-Johnson syndrome, toxic epidermal necrolysis;
  • other reactions: often - fatigue, feeling unwell, headache; very rarely - increased hair loss, exacerbation of psoriasis.

In places where the Atifin cream is applied, the following side effects may occur: burning or itching sensation, redness, allergic reactions.

Already in the first few days of taking Atifin, a decrease in the severity of the clinical symptoms of the disease is noted, however, treatment should be continued, since premature discontinuation of therapy or irregular use of the drug can lead to a relapse of the infection.

With onychomycosis, Atifin in the form of tablets is prescribed only in severe cases, when the majority of the nails are affected, pronounced subungual hyperkeratosis is noted, and previous local therapy is ineffective. The clinical response is observed several months after the cure of the fungus and the termination of the therapeutic course, which is associated with the rate of regrowth of a healthy nail. With a 3-week treatment of onychomycosis of the hands and 6-week treatment of onychomycosis of the feet, removal of the nail plates is not required.

During the period of use of the drug Atifin in the form of tablets, it is necessary to control the serum concentration of liver transaminases. If symptoms of impaired liver function appear (loss of appetite, jaundice, persistent nausea, abdominal pain, colorless stools, dark urine, weakness), the drug should be canceled.

Atifin cream is intended exclusively for external use. In case of contact with eyes, rinse immediately with running water. In case of serious eye irritation, consult a doctor.

Allergic reactions that occur during treatment are an indication for discontinuation of the drug.

During the period of treatment, it is necessary to follow the rules of hygiene to prevent possible re-infection through shoes and underwear. 2 weeks after the start of therapy, as well as at the end of treatment, antifungal treatment of stockings, socks and shoes should be carried out.

Drug interactions

When applied topically in the form of a cream, the likelihood of drug interactions is minimal.

With systemic use, Atifin inhibits the metabolism of beta-blockers, tricyclic antidepressants, antiarrhythmic drugs, selective catecholamine reuptake inhibitors, antipsychotics and type B monoamine oxidase inhibitors.

Rifampicin can speed up the elimination of terbinafine, cimetidine can slow it down.

With the simultaneous use of Atifin tablets with oral contraceptives, disruptions in the menstrual cycle are possible; with ethanol and hepatotoxic drugs - the likelihood of a hepatotoxic effect increases; with caffeine - the clearance of caffeine decreases by 21% and its half-life increases by 31%; with inhibitors / inducers of isoenzymes of the cytochrome P 450 system - sometimes a dose adjustment of Atifin is required.
