Pharmacological properties
Pharmacodynamics
Chondroitin sulfate is the main component of proteoglycans, which form a cartilage matrix in combination with collagen fibers. It has chondroprotective efficacy; inhibits the activity of enzymes that cause degradation of articular cartilage; improves metabolic processes in cartilage and subchondral bone; stimulates the production of proteoglycans by chondrocytes; affects phosphorus-calcium metabolism in cartilage tissue, stimulating its regeneration and participating in the construction of the basic substance of bone and cartilage tissue. The drug has an analgesic and anti-inflammatory effect, helping to reduce the release of inflammatory mediators and pain factors into the synovial fluid through macrophages and synoviocytes of the synovial membrane, suppressing the secretion of leukotriene B4 and prostaglandin E2.
Chondroitin sulfate prevents the collapse of connective tissue, stimulates the restoration of the cartilaginous surfaces of the joints, normalizes the production of joint fluid, thereby improving joint mobility and helping to reduce the intensity of pain and concomitant inflammation, and also reduces the need for non-steroidal anti-inflammatory drugs (NSAIDs). With degenerative changes in cartilage tissue, the drug is a means of replacement therapy.
The effect of the drug is felt within 2-3 weeks after application, the intensity of pain in the joints decreases, the clinical manifestations of reactive synovitis disappear, and the range of motion in the affected joints increases. After the end of therapy, the effect persists for 3–6 months.
Pharmacokinetics
The drug is easily absorbed by intramuscular injection, reaching the maximum concentration after 60 minutes, which then gradually decreases over 48 hours.
The drug accumulates mainly in the cartilage tissue of the joints, overcoming the synovial membrane. In the synovial fluid, chondroitin sulfate is detected 15 minutes after injection, then penetrates into the articular cartilage, where it reaches its maximum concentration after 2 days.
Indications for use
Lyophilisate
Chondroitin sulfate in the form of a lyophilisate is indicated for use in osteoarthritis of the peripheral joints and spine.
Solution
Chondroitin sulfate in the form of a solution is indicated for degenerative-dystrophic diseases of the joints and spine:
- osteoarthritis of the peripheral joints;
- intervertebral osteochondrosis and osteoarthritis.
The drug is also used to accelerate the formation of calluses in fractures.
Contraindications
- bleeding, bleeding tendency, thrombophlebitis;
- pregnancy and lactation;
- hypersensitivity to any component of the drug.
An additional contraindication for the use of the drug in the form of a solution is childhood.
Instructions for use Chondroitin sulfate: method and dosage
Chondroitin sulfate is administered intramuscularly. To prepare a solution from the lyophilisate, immediately before injection, dissolve the contents of the ampoule in 1 ml of water for injection.
Recommended dosage: 1 ml every other day. Starting from the 4th injection, in case of good tolerance, the dose is increased to 2 ml. The duration of therapy is 25-30 injections. Repeated courses are possible after 6 months. Their duration should be established by a medical professional.
To achieve a stable clinical effect, at least 25 injections of the drug are required, in which case its effect persists for a long time after application. Repeated courses are indicated in order to prevent exacerbations.
For the formation of callus, the drug in the form of a solution should be used 3-4 weeks every other day.
Side effects
- allergic reactions;
- hemorrhages at the injection site.
If these undesirable effects occur, the drug is recommended to be canceled.
Overdose
At the moment, there are no reports of cases of overdose of chondroitin sulfate.
special instructions
Influence on the ability to drive vehicles and complex mechanisms
According to the instructions, Chondroitin sulfate does not affect the concentration of attention and the speed of psychomotor reactions, therefore, the patient's ability to drive vehicles and other complex mechanisms during therapy does not change.
Application during pregnancy and lactation
There is no data on the safety and efficacy of the drug by pregnant and lactating women. It is recommended to refrain from drug therapy during these periods.
If it is necessary to use Chondroitin sulfate during lactation, breastfeeding should be interrupted.
Pediatric use
There are no data on the safety and efficacy of the drug in children.
Drug interactions
During drug therapy, one should be wary of the simultaneous use of direct anticoagulants.
Combined use with Chondroitin sulfate can enhance the effect of indirect anticoagulants, fibrinolytics and antiplatelet agents, which requires more frequent monitoring of blood clotting time during therapy.
Analogs
The analogues of Chondroitin sulfate are Artogistan, Artra, Artravir, Artradol, Artrafik, Drastop, Mucosat, Chondroguard, Chondroitin, Chondroitin-AKOS, Chondroxide, Chondrolone.
