Release form and composition

Dosage form - eye drops: a transparent solution of red (10 ml each in polyethylene dropper bottles, 1 bottle in a cardboard box).

Active ingredients contained in 1 ml of solution:

• Nicotinamide - 20 mg;
• Adenosine - 2 mg;
• Cytochrome C - 0.675 mg.

Additional components: water for injection, sorbitol, sodium hydrogen phosphate dihydrate, sodium succinate hexahydrate, sodium dihydrogen phosphate dihydrate, benzalkonium chloride.

Pharmacological properties

Pharmacodynamics

Oftan Katakhrom is a combined ophthalmic agent that improves the energy metabolism of the lens. Its action is due to the properties of the constituent components:

• nicotinamide stimulates the synthesis of nicotinamide dinucleotide and cofactor of dehydrogenases, is a part of the main metabolites of the eye crystal, coenzymes of nicotinamide adenine dinucleotide phosphate (NADP) and nicotinamide adenine dinucleotide (NAD), with an increase in the amount of which the rate of cataract development decreases;
• adenosine is a precursor of the angiotensin-converting enzyme and is involved in the metabolic processes of the lens. Expands blood vessels, helps restore microcirculation in the eye, normalizes blood oxygen saturation and outflow of toxins. Stimulates A2-receptors on the surface of cell membranes, thereby inhibiting inflammatory processes in the cornea, conjunctiva, anterior chamber of the eye. As a structural component of NADP and the enzyme glutathione reductase, adenosine plays the role of an intermediary in the processes of glutathione reduction in the eye crystal;
• cytochrome C is an antioxidant that neutralizes the action of free radicals on the cornea of the eye. Plays an important role in biochemical redox processes in the tissues of the eyeball. Acting directly on the crystal of the eye, the substance inhibits the synthesis of the enzyme cytochrome oxidase, thereby reducing the risk of developing cataracts.

Oftan Katakhrom is a low-toxic drug, therefore it can be used not only for the treatment of cataracts, but also for its prevention.

The drug affects not only the crystal. It was found that it has an antioxidant, antibacterial, anti-inflammatory and disinfectant effect on the surface of the eye, and is able to arrest the development of an infectious disease in the anterior part of the eye. Well moisturizes, relieves eye fatigue.

Pharmacokinetics

Nicotinamide is rapidly absorbed after the instillation of the drug Oftan Katahrom. Partially metabolized to nicotinic acid, then both compounds are methylated to form N-methylnicotinamide, which is further broken down in the liver. Nicotinamide in unchanged form and its metabolites are excreted by the kidneys in the urine.

Adenosine penetrates deep into the cornea and is distributed throughout all tissues, where it is very rapidly metabolized to form urate, xanthine and inosine, which are excreted through the kidneys in the urine. A part of the adenosine molecule is ribose - it is metabolized first to glyceraldehyde-3-phosphate, then to pyruvate, and is finally cleaved in the Krebs cycle. The plasma half-life is less than 1 minute.

When applied topically, cytochrome C is not absorbed into the systemic circulation. It is completely metabolized in the same ways as amino acids, after which it is broken down to bilirubin, which is excreted in the bile.

Indications for use

According to the instructions, Oftan Katakhrom is a drug for the treatment of cataracts of various origins.

Contraindications

Absolute:

• children and adolescents up to 18 years old;
• known hypersensitivity to any component of the ophthalmic agent.

During pregnancy and during breastfeeding, the drug is used as directed by a specialist, being careful.

Instructions for the use of Oftan Katakhrom: method and dosage

Oftan Katahrom is prescribed 1-2 drops into the conjunctival sac of the affected eye 3 times a day.

Before using the bottle for the first time, turn the cap while pressing it down.

Side effects

• From the side of the organ of vision: contact dermatitis, short-term tingling and burning sensation of the eyes, allergic conjunctivitis;
• Systemic reactions: short-term shortness of breath, dizziness, arterial hypotension and nausea; in some cases - a feeling of heat and pulsation in the temples, fainting.

Overdose

There is no information on a possible overdose. Given the insignificant systemic absorption of the drug and the rapid elimination of its active components from the body, overdose with topical application of eye drops is unlikely. If the solution is accidentally taken inside, symptomatic therapy should be carried out.

During treatment, you should not wear soft contact lenses, since the preservative contained in the preparation can be deposited in them and have an adverse effect on the tissues of the eyes.
