Bonderm is an antibacterial drug for external use.

Release form and composition

Dosage form Bonderm is an ointment for external use: a translucent white mass (15 g each in an aluminum tube, in a cardboard box 1 tube).

1 g of ointment contains:

  • active substance: mupirocin - 0.02 g;
  • auxiliary components: macrogol 400, macrogol 3350.

Pharmacological properties

Pharmacodynamics

Mupirocin is a topical antibiotic produced by fermentation of Pseudomonas fluorescens. It inhibits isoleucyl transfer RNA synthetase, disrupting the process of protein synthesis in the bacterial cell. The active ingredient Bonderm has bacteriostatic properties, which are manifested even in minimal inhibitory concentrations, and bactericidal properties characteristic of it when applied topically at higher concentrations.

Since mupirocin has a unique chemical structure and specific mechanism of action, no cross-resistance with other antibiotics has been reported.

According to the results of in vivo studies, mupirocin is active against beta-hemolytic strains Streptococcus spp., Staphylococcus epidermidis and Staphylococcus aureus (including strains resistant to methicillin).

In vitro, the effectiveness of the drug has been proven against the following microorganisms:

  • gram-negative aerobes (especially those causing infectious diseases of the skin): Pasteurella multocida, Haemophilus influenzae, Moraxella catarrhalis, Neisseria meningitidis, Neisseria gonorrhoeae;
  • gram-positive aerobes: Streptococcus spp., Staphylococcus epidermidis (including strains synthesizing beta-lactamases and strains resistant to methicillin), Staphylococcus aureus (including strains producing beta-lactamases and methicillin-resistant strains) coagulase-negative staphylococci, including strains resistant to methicillin.

The following bacteria are insensitive to the action of mupirocin: anaerobes, Corynebacterium spp., Micrococcus spp., Enterobacteriaceae, gram-negative non-fermenting bacilli. The resistance range is 0 to 23%.

Pharmacokinetics

Mupirocin is practically not absorbed through intact skin. In the case of penetration through damaged skin, this substance is rapidly metabolized to form monic acid, a metabolite that completely lacks microbiological activity.

Mupirocin is excreted at a high rate from the body in the urine as inactive monic acid.

Indications for use

According to the instructions, Bonderm is indicated for the treatment of primary and secondary infectious skin lesions caused by microorganisms sensitive to mupirocin:

  • primary skin infections: pyoderma (staphyloderma, streptoderma), including folliculitis, impetigo, sycosis, furunculosis, furuncles of the auricle and external auditory canal, ecthyma;
  • secondary infections: infected injuries (insect bites, cuts, abrasions, scratches), infected dermatitis (allergic, atopic or contact), infected eczema, trophic ulcers, burns or wounds that do not require hospitalization.

In addition, the ointment is used to prevent bacterial lesions and related complications in small wounds, cuts or bedsores.

Contraindications

  • age up to 3 years;
  • individual intolerance to the components of the drug.

Bonderm ointment should be used with caution in patients with renal insufficiency.

During pregnancy and breastfeeding, the drug can be used only in case of urgent need, when the expected effect of therapy for the mother exceeds the possible threat to the fetus or child.

Instructions for the use of Bonderm: method and dosage

Bonderm ointment is intended for external use by applying a sufficient amount to the affected skin. If necessary, you can use an aseptic or occlusive dressing.

Recommended dosage regimen: 2-3 application procedures per day, the duration of treatment depends on the severity of the lesion and should be no more than 10 days.

If, after five days of therapy, there is no clinical effect, it is recommended to consult a doctor to revise the diagnosis, taking into account clinical indicators.

When treating children, the elderly and patients with impaired liver function, dose adjustment is not required.

Side effects

  • allergic reactions: rarely - edema at the site of application, rash; in isolated cases - generalized allergic reactions;
  • others: very rarely - dry skin at the site of application, burning, itching.

Overdose

Mupirocin practically does not show toxic properties. There are currently no reports of Bonderm overdose cases. With a diagnosed overdose of mupirocin, the specific treatment is unknown. In this case, supportive therapy is recommended and, if necessary, appropriate monitoring of the patient's condition. In patients with renal insufficiency, renal function should be monitored, which is associated with possible adverse reactions caused by polyethylene glycol.

In severe chronic renal failure, the ointment should be applied to large areas of the skin with caution, since this causes great stress on the kidneys due to high doses of absorbed substances.

Avoid contact of the ointment with mucous surfaces of the body, including the eyes. If the ointment gets on the mucous membranes, they should be thoroughly rinsed with running water.

With significant irritation of the skin or the development of allergic reactions, the use of Bonderm should be discontinued, carefully remove the remnants of the ointment from the affected area and prescribe another antibacterial therapy.

Bonderm does not affect the patient's ability to drive vehicles and mechanisms.

Drug interactions

The simultaneous use of other drugs for external use can reduce the antibacterial effect or disrupt the stability of the active substance of the ointment, so any combination should be avoided.
