Release form and composition
Dosage form - film-coated tablets: capsule-shaped, blue, on one side - transverse risk, on the other - engraving "SECN" (in a cardboard box, instructions for use of Seknidox and 2 tablets packed in a blister).
Composition of the preparation for 1 film-coated tablet:
- active substance: secnidazole - 1000 mg;
- auxiliary components: magnesium stearate, sodium carboxymethyl starch, corn starch, povidone K30, calcium hydrogen phosphate, microcrystalline cellulose, talc;
- film shell: propylene glycol, talc, titanium dioxide (E171), hypromellose E-15, aluminum varnish based on indigo carmine (E132).
Pharmacological properties
Pharmacodynamics
The active ingredient of Seknidox - Seknidazole is a derivative of nitroimidazole with bactericidal (against anaerobic bacteria) and amoebicidal (intra- and extraintestinal) action. It is especially active against Entamoeba histolytica, Giardia lamblia and Trichomonas vaginalis.
The mechanism of action of secnidazole is based on its ability to interact with cellular DNA, since after penetration into the cell, the drug is activated due to the reduction of the 5-nitro group. As a result, the spiral structure of the bacterial DNA is disturbed, its threads are destroyed, nucleotide synthesis is inhibited - all this leads to cell death.
Pharmacokinetics
When taken orally from the gastrointestinal tract, secnidazole is rapidly absorbed. Its bioavailability reaches 80%. After applying a dose of 2000 mg, the maximum plasma concentration (C max) is observed after 3-4 hours.
The drug is able to penetrate the blood-brain and placental barriers, as well as into breast milk.
It is excreted from the body slowly: in 120 hours, about half of the dose received. Excretion occurs mainly in the urine.
Indications for use
- giardiasis caused by Giardia lamblia;
- amoebiasis of the liver or intestines, the causative agent of which is the amoeba Entamoeba histolytica;
- urethritis and vaginitis provoked by Trichomonas vaginalis microorganisms.
Contraindications
Absolute:
- children under 3 years of age or body weight less than 16 kg (due to the high dosage of the drug);
- period of pregnancy and lactation;
- blood disorders (currently or in medical history);
- organic diseases of the central nervous system;
- increased sensitivity to any component of Seknidox.
Seknidox, instructions for use: method and dosage
Seknidox tablets are taken orally right before meals. They should be swallowed whole with plenty of water.
Recommended doses depending on indications:
- liver amebiasis: adults - 1500 mg / day (1 ½ tablets), children - 30 mg / kg / day. The dose can be taken all at once or divided into several doses. The course of treatment is 5 days;
- intestinal amebiasis without clinical symptoms, including focal and cystic forms: adults - 2000 mg / day (2 tablets), children - 30 mg / kg / day. The dose can be taken all at once or divided into several doses. The duration of therapy is 3 days;
- acute amebiasis with severe clinical symptoms (histolytica form): adults - 2000 mg once, children - 30 mg / kg once;
- vaginitis and urethritis: adults - 2000 mg once;
- giardiasis: children - 30 mg / kg once.
Side effects
- from the digestive system: a feeling of metal taste in the mouth, stomatitis, glossitis, pain in the stomach, nausea, vomiting;
- on the part of the blood: moderate reversible leukopenia;
- from the nervous system: decreased skin sensitivity, impaired coordination of movements, dizziness, peripheral neuropathy;
- allergic reactions: rash, urticaria, angioedema; very rarely - anaphylaxis.
Overdose
When the recommended doses are exceeded, there is an increase in the severity of dose-dependent side effects.
In case of overdose, Secnidox should be canceled. It is necessary to induce vomiting, take activated charcoal and urgently consult a doctor. Treatment is symptomatic.
special instructions
Patients with hepatic amebiasis in the suppuration phase, in addition to taking Secnidox, require removal of abscesses or drainage of pus.
During the entire period of taking Secnidox and within 5 days after the end of therapy, alcoholic beverages and ethanol-containing drugs should not be consumed.
With long-term treatment, it is recommended to control the blood count. If leukopenia is detected, the advisability of continuing to take the drug is determined individually after the ratio of the risk of worsening leukopenia with the risk of spreading the infectious process. Patients with a history of blood diseases should avoid the appointment of Seknidox.
Cancellation of the drug is required for patients who develop dizziness or ataxia during therapy, or other deterioration of neurological status is observed.
With the combined use of indirect anticoagulants, more frequent determination of prothrombin time and control of the International Normalized Ratio are required. It is necessary to take into account the likelihood that an anticoagulant dose adjustment will be required during the period of taking Seknidox and within 8 days after its cancellation.
Women with Trichomonas vaginitis and men with Trichomonas urethritis must undergo treatment at the same time as their sexual partner. For the period of therapy, abstinence from sexual intercourse is indicated. A woman's menstruation is not a reason to interrupt treatment.
Influence on the ability to drive vehicles and complex mechanisms
Seknidox can interfere with motor coordination and cause dizziness. At the beginning of treatment, before establishing an individual response to the drug, it is important to be careful when driving and working with complex mechanisms. In the event of the occurrence of these reactions, one should refrain from engaging in potentially hazardous activities.
Application during pregnancy and lactation
In animal studies, the teratogenic effects of secnidazole have not been identified. However, the existing experience of clinical use of the drug in humans during pregnancy is insufficient to assess its possible fetotoxic effect. In addition, it has been found that the drug crosses the placental barrier, and the results of animal studies are not always comparable to the reactions in humans. In this regard, Seknidox is contraindicated for use in pregnant women.
Secnidazole passes into breast milk. There are known cases of diarrhea, candidiasis of the anal region and oral mucosa in an infant. For this reason, you should refrain from breastfeeding during treatment with Secnidox tablets.
Pediatric use
In this dosage form, secnidazole is contraindicated in children under 3 years of age or weighing less than 16 kg.
Drug interactions
With the simultaneous administration of secnidazole and disulfiram, there is a high risk of confusion and the development of psychotic reactions.
Seknidox enhances the effect of oral anticoagulants (warfarin, indandione, coumarin derivatives), reducing their hepatic metabolism, thereby increasing the risk of bleeding.
Drinking alcohol or taking ethanol-containing drugs during therapy with secnidazole causes symptoms of an antabuse-like reaction: hot flashes, headache, tachycardia, nausea, abdominal cramps, vomiting.
