Minirin is a vasopressin-like drug used in the treatment of diabetes insipidus.

Release form and composition

Minirin is available in the following dosage forms:

  • Tablets: white, convex, oval; on one side - risk, on the other - the inscription (depending on the dosage) "0.1" or "0.2" (30 pcs. in plastic bottles, 1 bottle in a cardboard box);
  • Sublingual tablets: round, white; on one side - marking in the form of 1, 2 or 3 drops (depending on the dosage of the active substance, 0.06 / 0.12 / 0.24 mg, respectively) (10 pcs. in blisters, 1, 3 or 10 blisters in a cardboard box).

The composition of 1 tablet includes:

  • Active ingredient: desmopressin - 0.1 or 0.2 mg (in the form of desmopressin acetate);
  • Auxiliary components: lactose, povidone, potato starch, magnesium stearate.

The composition of 1 sublingual tablet includes:

  • Active ingredient: desmopressin - 0.06, 0.12 or 0.24 mg (in the form of desmopressin acetate - 0.067, 0.135 or 0.27 mg, respectively);
  • Auxiliary components: gelatin - 12.5 mg, mannitol - 10.25 mg, citric acid - up to pH 4.8.

Indications for use

  • Central diabetes insipidus;
  • Nocturnal polyuria (nocturia) in adults (as symptomatic treatment);
  • Primary nocturnal enuresis in children from 5 years of age (for tablets) or 6 years (for sublingual tablets).

Contraindications

Absolute:

  • Heart failure and other conditions that require the use of drugs with a diuretic effect;
  • Hyponatremia;
  • Renal failure, proceeding in severe and moderate (with creatinine clearance <50 ml per minute);
  • Syndrome of inadequate production of antidiuretic hormone (Parkhon's syndrome);
  • Polydipsia (psychogenic or habitual);
  • Age up to 6 years (for sublingual tablets);
  • Hypersensitivity to drug components.

Relative (the appointment of Minirin requires caution):

  • Renal failure (with creatinine clearance> 50 ml per minute);
  • Fibrosis of the bladder;
  • Violations of water and electrolyte balance;
  • The potential for increased intracranial pressure;
  • Pregnancy;
  • Age up to 1 year (for tablets);
  • Age from 65 years (due to the high likelihood of side effects, including fluid retention, hyponatremia; when prescribing the drug before starting therapy, 3 days after starting it, and with each dose increase, it is necessary to monitor the patient's condition and determine the level of sodium in plasma blood).

Method of administration and dosage

The doctor determines the regimen for taking the drug individually.

Dosage ratio between dosage forms of Minirin (tablets - sublingual tablets):

  • 0.1 mg - 0.06 mg;
  • 0.2 mg - 0.12 mg;
  • 0.4 mg - 0.24 mg.

It is advisable to take the drug after meals (to avoid the influence of food on its absorption and effectiveness).

Depending on the dosage form Minirin is taken:

  • Tablets: inside with liquid;
  • Sublingual tablets: sublingual (you cannot drink the tablets with liquid).

Recommended dosing regimen (in tablet doses):

  • Central diabetes insipidus (children and adults): initial single dose - 0.1 mg, frequency of administration - 1-3 times a day. In the future, depending on the response to the treatment, it is possible to adjust the dose. In most cases, the daily dose is in the range of 0.2-1.2 mg; the optimal maintenance single dose is 0.1-0.2 mg with a frequency of administration 1-3 times a day;
  • Primary nocturnal enuresis: the initial dose is 0.2 mg, the drug is recommended to be taken at night. With insufficient effectiveness, it is possible to increase the dose by 2 times. It is necessary to limit fluid intake in the evening. The recommended duration of the course of continuous therapy is 3 months. Based on clinical data (after discontinuation of the drug within 1 week), a decision can be made to continue treatment;
  • Nocturnal polyuria (adults): initial dose - 0.1 mg, the drug is recommended to be taken at night. In case of insufficient effectiveness within 1 week, it is possible to increase the dose by 2-4 times (the dose can be increased no more than 1 time per week).

While taking Minirin, you need to remember about the danger of fluid retention in the body.

If after a month of therapy an adequate clinical effect is not observed, further use of the drug is not recommended.

Side effects

Most often, side effects are observed in cases where therapy is carried out without restricting fluid intake, which leads to fluid retention and / or hyponatremia. Violations can be asymptomatic or be accompanied by the following symptoms:

  • Digestive system: vomiting, nausea, dry mouth;
  • Central and peripheral nervous system: dizziness, headache; in especially severe cases - convulsions;
  • Others: weight gain, peripheral edema.

To avoid the development of side effects, in patients with primary nocturnal enuresis, it is imperative to limit fluid intake to a minimum 1 hour before and for 8 hours after taking Minirin.

According to clinical studies, most often side effects occur in elderly patients (over the age of 65).

If there are other additional causes for fluid retention and electrolyte disturbances, the use of Minirin is not recommended.

Prevention of the development of hyponatremia consists in increasing the frequency of sodium determination in blood plasma, especially when combined with drugs that lead to the appearance of a syndrome of inadequate secretion of antidiuretic hormone (including selective serotonin inhibitors, tricyclic antidepressants, carbamazepine, chlorpromazine), and nonsteroidal anti-inflammatory drugs.

In case of nocturia and / or dysuria, acute urinary incontinence, urinary tract infections, suspicion of a tumor of the prostate gland or bladder, the presence of decompensated diabetes mellitus and polydipsia, the diagnosis and treatment of these diseases / conditions should be carried out prior to the use of Minirin.

If during treatment systemic infections, fever, gastroenteritis develop, the drug should be discontinued.

Drug interactions

With the combined use of Minirin with some drugs, the following effects may be observed:

  • Loperamide (presumably - and other drugs that slow down peristalsis): a significant increase in the level of desmopressin in plasma, which leads to an increased risk of fluid retention and the development of hyponatremia;
  • Indomethacin: strengthening, but not increasing the duration of action of Minirin;
  • Non-steroidal anti-inflammatory drugs: increased risk of side effects;
  • Norepinephrine, glybutide, lithium, tetracycline: weakening the antidiuretic action of Minirin;
  • Hypertensive drugs: enhancing their action;
  • Carbamazepine, chlorpromazine, selective serotonin inhibitors, tricyclic antidepressants: development of Parkhon's syndrome, increased antidiuretic effect of Minirin, increased risk of hyponatremia and fluid retention;
  • Dimethicone: decreased absorption of desmopressin.
