Calcium gluconate-SOLOpharm is a calcium preparation that compensates for the deficiency of calcium ions, which are necessary for synaptic transmission of nerve impulses, contraction of skeletal and smooth muscles, cardiac muscle function, blood coagulation, osteogenesis (the process of bone tissue formation).
When administered intravenously, the drug stimulates the sympathetic part of the autonomic nervous system, increases the synthesis of adrenaline by the adrenal glands.
Pharmacokinetics
Parenteral administration of calcium gluconate ensures an even distribution of the substance with the blood flow in all tissues and organs. Calcium in blood plasma is found both in bound form, in a complex with proteins and anions of organic / inorganic acids (bicarbonate, lactate, calcium citrate, phosphate), and in the form of free ions.
Calcium passes through the placental barrier, penetrates into breast milk during lactation.
It is excreted from the body mainly by the kidneys.
Indications for use
Calcium gluconate-SOLOpharm is used for the treatment of acute hypocalcemia.
The drug is also used to reduce capillary permeability associated with allergic conditions, non-thrombocytopenic purpura and exudative dermatoses, for example, with dermatitis herpetiformis and maculopapular rash due to drug allergies.
Contraindications
Absolute:
- hypercalciuria;
- hypercalcemia, including in patients with hypervitaminosis D, hyperparathyroidism, renal failure, decalcifying malignant neoplasms, sarcoidosis, osteoporosis due to immobilization, Burnett's syndrome (lactic-alkaline syndrome);
- poisoning with cardiac glycosides;
- use simultaneously with therapy with cardiac glycosides;
- children's age - for intramuscular injection;
- hypersensitivity to calcium gluconate and auxiliary components of the drug.
With caution, after consultation with a specialist, Calcium gluconate-SOLOpharm is used in patients with dehydration, electrolyte disturbances (hypercalcemia may develop), diarrhea, malabsorption syndrome, a history of calcium nephrourolithiasis, minor hypercalciuria, moderate hyperosclerosis, chronic renal atherosclerosis, in old age (especially in the presence of one or more of the conditions listed above), with nephrocalcinosis, pathology of the cardiovascular system, sarcoidosis, chronic heart failure, the simultaneous use of epinephrine.

Calcium gluconate-SOLOpharm solution is intended for parenteral administration by intravenous or intramuscular administration. Calcium-containing solutions must be injected slowly to minimize peripheral vasodilation and cardiac depression. Before administration, the drug should be heated to body temperature.
The rate of total calcium in the blood serum is 2.25-2.75 mmol / l (4.45-5.5 sEq / l). The introduction of calcium gluconate is intended to ensure the restoration of normal levels of calcium in the blood plasma.
Recommended dosage of Calcium gluconate-SOLOpharm for adult patients: the solution is injected deeply intramuscularly, slowly intravenously (for 2-3 minutes) or intravenously drip in a dose of 5-10 ml 100 mg / ml (10% solution), daily, every other day or once every two days (taking into account the clinical condition of the patient and the nature of the disease). Subsequent dosing is determined by taking into account the serum calcium concentration in the blood.
For children and adolescents under 18 years of age, the dose and route of administration is prescribed depending on the level of development of hypocalcemia, as well as the nature and severity of symptoms.
Recommended dosage of Calcium gluconate-SOLOpharm for children, depending on age [10% solution (100 mg / ml) is injected intravenously]:
- up to 6 months - 0.1-1 ml;
- from 7 to 12 months - 1-1.5 ml;
- from 1 to 3 years - 1.5–2 ml;
- from 4 to 6 years old - 2–2.5 ml;
- from 7 to 14 years old - 3.5 ml;
- over 14 years old - dosage as for adults.
It is forbidden for children to injections of Calcium gluconate-SOLOpharm into the muscle, as this can lead to the development of necrosis. It is recommended to carry out a slow intravenous injection or intravenous infusion after dilution of the solution (to reduce the rate of administration and exclude the development of local irritation or necrosis due to its accidental ingress into the perivascular tissues).
For intravenous infusion, the solution is diluted in a ratio of 1 ÷ 10, providing a concentration of 10 mg / ml, using 0.9% sodium chloride solution or 5% glucose solution. Overdose
Symptoms of calcium overdose (hypercalcemia) are anorexia, abdominal pain, nausea, vomiting, constipation, dehydration, polyuria, polydipsia, bone pain, muscle weakness, kidney calcification, confusion, drowsiness, hypersomnia, increased blood pressure; with severe intoxication, arrhythmia (up to cardiac arrest) and coma are possible.
Therapy of the condition is aimed at reducing the elevated level of calcium in the blood plasma. At the initial stage, rehydration is carried out. In the case of severe hypercalcemia, in order to increase the amount of extracellular fluid, intravenous infusion of 0.9% sodium chloride solution may be required. Calcitonin is recommended to reduce calcium concentration. If it is necessary to accelerate the excretion of calcium, you can apply Furosemide. However, thiazide diuretics cannot be used, since they can enhance the absorption of calcium salt in the kidneys. Treatment of hypercalcemia is followed by careful monitoring of serum electrolyte concentrations in the blood.

- thiazide diuretics: can increase hypercalcemia;
- calcitonin: in the treatment of hypercalcemia, its effectiveness decreases;
- phenytoin: its bioavailability decreases.
