Pharmacological properties

Pharmacodynamics

Pentaflucin daily is a combined drug, the pharmacological activity of which is due to the complex effect of the ingredients that make up its composition.

Properties of the individual components of Pentaflucin daily:

  • paracetamol: has analgesic and antipyretic effects; acting on the centers of thermoregulation and pain, it blocks the enzymes COX-1 (cyclooxygenase-1) and COX-2 (cyclooxygenase-2);
  • ascorbic acid: takes part in the regulation of blood clotting and tissue regeneration, plays an important role in oxidation-reduction processes and carbohydrate metabolism, increases the body's resistance to infections;
  • caffeine: has psychostimulating properties (activates psychomotor centers in the brain), has an analeptic effect, eliminates the feeling of fatigue and drowsiness, increases mental and physical performance, enhances the analgesic effect of paracetamol;
  • rutoside: reduces capillary fragility and permeability, has antioxidant activity;
  • calcium gluconate: prevents increased fragility and permeability of blood vessels (due to these phenomena, hemorrhagic processes develop in acute respiratory viral infections and influenza), restores capillary blood circulation, has antiallergic effect (its mechanism is unclear), is a source of calcium.

Pharmacokinetics

Data on the pharmacokinetics of Pentaflucin daily are not provided.

Indications for use

Pentaflucin daily is used in adult patients for the purpose of symptomatic treatment of ARVI (acute respiratory viral infections) and ARI (acute respiratory diseases), which are accompanied by headache, chills, high fever and muscle pain.

Contraindications

  • severe renal and / or liver dysfunction;
  • deficiency of the enzyme glucose-6-phosphate dehydrogenase;
  • blood diseases characterized by a tendency to hemorrhage (hemophilia, thrombocytopenic purpura, etc.);
  • sucrase / isomaltase deficiency, glucose-galactose malabsorption syndrome, hereditary fructose intolerance;
  • children and adolescents up to 18 years old;
  • period of pregnancy and lactation;
  • hypersensitivity to the main or auxiliary components of the drug.

Pentaflucin daily, instructions for use: method and dosage

The drug is taken orally, after meals. The contents of the sachet are dissolved in 125 ml (1/2 cup) of hot, but not boiling water (the water temperature should be about 70 ° C). Slight sediment is acceptable. Shake the solution before use. Only freshly prepared solution is suitable for use.

The recommended dose of Pentaflucin daily is 1 sachet up to 3-4 times a day. The duration of the course of treatment is 3-5 days.

Side effects

If the recommended dosage regimen of Pentaflucin is observed, daytime side effects are rare. Headache, dizziness, nausea, vomiting, general weakness, skin allergic reactions are possible.

Undesirable side effects due to ascorbic acid:

  • digestive system: irritation of the mucous membrane of the gastrointestinal tract;
  • urinary system: when taking doses over 600 mg per day - moderate pollakiuria; with long-term treatment with high doses - kidney stones (calcium oxalate stones), hyperoxaluria, damage to the glomerular apparatus of the kidneys;
  • allergic reactions: skin redness, rash;
  • other reactions: glucosuria and hyperglycemia (due to inhibition of the function of the insular apparatus of the pancreas).

Side effects of Pentaflucin daily due to caffeine:

  • digestive system: vomiting, nausea, exacerbation of gastric ulcer and duodenal ulcer;
  • cardiovascular system: increased blood pressure, tachycardia, palpitations, heart rhythm disturbances;
  • nervous system: anxiety, restlessness, dizziness, insomnia, headache, agitation, rapid shallow breathing, tremors, increased reflexes.

Undesirable effects of Pentaflucin daily due to the content of paracetamol:

  • digestive system: epigastric pain, nausea;
  • hepatobiliary system: hepatotoxic effect (with long-term therapy with large doses);
  • hematopoietic system: thrombocytopenia, anemia, increased content of methemoglobin in the blood;
  • genitourinary system: nephrotoxic effect (with prolonged use in large doses);
  • allergic reactions: urticaria, skin rash, itching, Quincke's edema.

Side effects of the drug due to rutoside:

  • digestive system: heartburn, nausea, loose stools;
  • cardiovascular system: feeling of flushing of the face;
  • nervous system: headache;
  • allergic reactions: skin rash.

Calcium gluconate, which is also part of Pentaflucin daily granules, can cause gastrointestinal side effects such as irritation of the gastrointestinal tract mucosa and constipation.

Overdose

An overdose of Pentaflucin daily is associated with paracetamol intoxication. In the first 24 hours after an overdose, the patient develops nausea, vomiting and anorexia, pains in the abdomen, the skin becomes pale, glucose metabolism is disturbed, metabolic acidosis may develop. The first signs of liver dysfunction may appear 12–48 hours after taking the drug. In the case of a severe overdose, acute renal failure with tubular necrosis is observed (while severe liver damage may be absent), pancreatitis, heart rhythm disturbances, liver failure with encephalopathy, subsequent coma and even death. In adults, the hepatotoxic effect is manifested when taking 10 g of paracetamol or more.

If an overdose is suspected, the patient's stomach is washed and intestinal sorbents are prescribed (for example, activated carbon). The patient should definitely seek medical attention. During the first 8-9 hours, the victim is injected with acetylcysteine, methionine and SH-group donors. Further therapy depends on the content of paracetamol in the blood and the patient's condition.

special instructions

Taking Pentaflucin daily is not recommended to be combined with the use of alcoholic beverages, since ethanol increases the risk of toxic liver damage.

Influence on the ability to drive vehicles and complex mechanisms

During treatment, care must be taken when performing work that requires a high concentration of attention and a quick reaction.

Application during pregnancy and lactation

Pentaflucin daily is contraindicated in pregnant and lactating women. If the use of the drug during breastfeeding is necessary, you should stop breastfeeding for the entire period of therapy.

Pediatric use

Pentaflucin daily is not used in pediatric practice (contraindicated in children and adolescents under 18 years of age).

With impaired renal function

The drug should not be administered to patients with severe renal impairment.

For violations of liver function

Severe liver dysfunctions are a contraindication to the use of Pentaflucin daily.

Drug interactions

Pentaflucin daily is not recommended to be combined with phenytoin, rifampicin, barbiturates, carbamazepine, zidovudine, anticonvulsants and other inducers of liver microsomal enzymes.

Drug interactions associated with the content of paracetamol in the preparation:

  • anticoagulants: when using large doses of paracetamol, the effect of anticoagulant drugs is enhanced (due to a decrease in hepatic synthesis of procoagulant factors);
  • diflunisal: the concentration of paracetamol in plasma increases (by about 50%), and the risk of hepatotoxic action increases;
  • ethanol: the likelihood of acute pancreatitis increases;
  • uricosuric drugs: the effectiveness of uricosuric drugs decreases;
  • myelotoxic drugs: the hematotoxicity of paracetamol increases;
  • other non-steroidal anti-inflammatory drugs: with prolonged joint use with paracetamol, the risk of papillary necrosis of the kidneys and analgesic nephropathy increases, as well as the likelihood of end-stage renal failure.

Caffeine-mediated interactions:

  • nicotine: the rate of caffeine elimination increases;
  • mexiletine: reduces the excretion of caffeine by up to 50%;
  • cardiac glycosides: absorption is accelerated and the action of glycosides is enhanced, and their toxicity also increases;
  • norfloxacin, ciprofloxacin, oral contraceptives and cimetidine: hepatic metabolism of caffeine slows down and its concentration in the blood increases;
  • monoamine oxidase inhibitors, including selegiline, furazolidone, procarbazine: in combination with large doses of caffeine, a marked increase in blood pressure or the development of dangerous arrhythmias is possible;
  • theophylline and other xanthines: it is possible to reduce the clearance of theophylline and other xanthines and increase their toxic effects;
  • adrenergic bronchodilator drugs: additional stimulation of the central nervous system may occur;
  • beta-blockers: mutual suppression of therapeutic effects is possible.

Drug interactions of Pentaflucin daily associated with ascorbic acid:

  • indirect anticoagulants and heparin: their effectiveness decreases;
  • acetylsalicylic acid: the excretion of ascorbic acid in the urine increases, and the excretion of acetylsalicylic acid slows down; the absorption of ascorbic acid is reduced by about 30%;
  • tetracyclines, benzylpenicillin: the plasma concentration of tetracyclines and benzylpenicillin increases;
  • ethinyl estradiol: when using ascorbic acid at a dose of 1 g per day, the bioavailability of ethinylestradiol increases;
  • iron preparations: the intestinal absorption of iron-containing preparations improves;
  • rutoside: the pharmacological action of rutoside is enhanced;
  • isoprenaline: the chronotropic effect of isoprenaline decreases;
  • short-acting sulfonamides, salicylates: the risk of crystalluria increases;
  • oral contraceptives: the plasma concentration of oral contraceptives decreases (the latter, in turn, slow down the absorption and assimilation of ascorbic acid).

Ascorbic acid increases the rate of elimination of drugs that have an alkaline reaction and slows down the renal excretion of acids. Alkaline drinking and fresh juices impair the absorption of ascorbic acid and its assimilation.

Drug Interactions Associated with Calcium Gluconate:

  • digoxin, tetracyclines, iron preparations for oral administration: absorption of the listed agents slows down (it is recommended to observe the interval between taking calcium gluconate and these drugs for at least 2 hours);
  • quinidine: an increase in the toxicity of quinidine and inhibition of intraventricular conduction is possible;
  • tetracycline antibiotics: the antibacterial effect of antibiotics decreases (since calcium gluconate forms insoluble complexes with them);
  • phenytoin: the bioavailability of phenytoin decreases;
  • thiazide diuretics: increased hypercalcemia is possible;
  • calcitonin: decreases the effectiveness of calcitonin used in hypercalcemia;
  • blockers of slow calcium channels: the therapeutic effect of calcium channel blockers decreases;
  • sulfates, salicylates and carbonates: calcium gluconate is incompatible pharmaceutically with the listed groups of drugs, since it forms sparingly soluble or insoluble calcium salts when interacting with them.
